Summary:
The primary purpose of this position is to provide clinical assessments, intensive case management, and referral program management. The incumbent will facilitate collaboration between the HACP, residents, and agencies and organizations serving HACP residents. Collaborate with HACP Property Managers, HACP Projects, Resident Self Sufficiency staff, and service providers to assist in resident self-sufficiency. The
incumbent is responsible for developing a preliminary treatment plan, monitoring linkage and progress, and completing required clinical documentation related to individual caseloads in a timely manner. All activities must support the Housing Authority of the City of Pittsburgh (“HACP” or “Authority”) mission, strategic goals and objectives.

Education and/or Experience
Master’s degree in Social Sciences, Counseling, Psychology Social Work or a related field and a minimum of 2 years’ experience working in clinical coordination, clinical consultant, therapist or social services. Will consider other equivalent combination of education and work experience, which will result in the required skills, knowledge and abilities. PA Driver’s License and use of an automobile during work hours required. Must be able to work evenings and weekend.
Technical Skills
To perform this job successfully, an individual must have strong computer skills and must be proficient in Microsoft Office applications (MS Word, Excel, and Outlook). Must have the ability to learn other computer software programs as required by assigned tasks. Must be proficient in completion of psychosocial assessments; must be able to make sound decisions, and sound customer service skills.
