Position Summary:
We have an exciting opportunity to join our team as a Clinical Coordinator, Rehab.

In this role, the successful clinical coordinator coordinates, co-manages, negotiates, and procures the care of pediatric patients by providing focused care coordination across the pediatric rehab continuum (acute, hospital outpatient based, community services). Evaluates appropriate clinical resource utilization, monitors and facilitates access to care, and assesses patients for transition of patient through continuum of care via review of patient records and communication with therapy and medical team. Uses patient and family-centered care models to build partnerships among pediatric providers, patients, families, and the community. Acts as a liaison with the health care team to ensure the achievement of quality service outcomes for patients and families.

Job Responsibilities:

• Collaborates with patient/family, physician(s), and health care team to align appropriate patient goals and treatment expectations to achieve optimal outcomes.
• Prioritizes, organizes, and facilitates referrals to multidisciplinary therapy services or physician specialty services in anticipation of, or in response to intervention outcomes.
• Pursues supporting care coordination documentation to further support treatment goals (e.g. specialty equipment, medical justification for services).

• Focuses the care plan on quality of life and effective utilization of resources to facilitate goal achievement and movement through the continuum of care.

• Performs screenings for pediatric population to identify health risks and coordinates appropriate referrals based on results.

• Serves as a resource and educates the care team and/or family around diagnosis-specific standards of care, relevant interventions, neurodevelopmental concerns, and community resources, through both formal and informal teaching strategies.

• Ensures appropriate therapeutic and medical resources are accessible, considering all barriers to care access and identified social determinants of health.

• Utilizes data-driven monitoring tools to evaluate care coordination effectiveness and design quality improvement strategies to further meet patient/family needs while incorporating HRO initiatives, and quality and safety goals.

• Utilizes evidence-based practice to further strategic initiatives promoting interdisciplinary healthcare outcomes.

• Develops procedural approaches to support the care planning needs for specific patient populations, identifying trends in care that may provide opportunity for improvement in clinical outcomes.

• Promotes own professional growth and development in care management role, identifying practice abilities/limitations and obtaining instruction and supervision as necessary.

• Achieves and maintains current professional licensure, national certification, and/or higher education in allied health profession. Provides only case coordination services within scope of practice and refers patient to another source for services outside scope of practice.

• Understands basic reimbursement systems and identifies potential payor issues relative to delays in treatment. Acts as an advocate/facilitator in cases with identified insurance barriers (e.g. participates in the peer-to-peer process), and assists in developing strategies to decrease avoidable days.

• Reports, escalates, and analyzes significant clinical and operational barriers, and addresses with the pediatric Associate Director and/or Site Director.

• Documents assessments, findings, progress, interventions, and recommendations in medical record according to established departmental and hospital policy standards.

• Supports the mission, philosophy, standards, goals and objectives of NYU Langone Health.

• Contributes to the development of the goals and objectives of pediatric Rusk Rehabilitation.

• Understands, applies and supports departmental/hospital policies, procedures and standards.

• Observes at all times legal and ethical considerations pertaining to patients and hospital personnel.

• Performs other duties as assigned.

Minimum Qualifications:
Bachelors degree in field of practice.Must hold a valid New York State license and current registration in clinical discipline (Physical Therapist, Occupational Therapist or Speech-Language Pathologist).Minimum of three (3) to five (5) years of licensed clinical experience with progressive responsibilities and development in clinical area of licensure.Strong communication and interpersonal skills.Current BLS certification (issued by the American Heart Association or the American Red Cross) or CPR/AED certification (issued by the American Heart Association or the American Red Cross) is required at the time of hire.

Required Licenses: Speech Pathologist Lic-NYS, Physical Therapist License-NYS, Occupational Therapist License-NYS, ASHA Certification

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $84,577.93 - $145,897.01 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
