Overview:
Join the Loretto Health & Rehabilitation as a Clinical Coordinator, RN!
Under the guidance of the Unit Nurse Manager, The Clinical Coordinator is responsible for coordinating the clinical practice and care to residents in a Long Term Care and Short Term Rehab setting. This role is a liaison between residents, families, medical providers and Loretto employees. Conducts patient assessments, implements and evaluates the effectiveness of nursing care delivered to the residents.
Salary: $84,323 - $94,702 Annually
Schedule: Monday - Friday- 08:00am-4:30 PM
Responsibilities:
Responsibilities include:
Oversee the delivery of Quality Patient Care
Compliance with NYSDOH and CMS regulations and facility policies and protocols
Orients, trains and mentors new employees
Provides health training to patients and their families
Assists in gathering and analyzing data for QAPI
Acts as primary liaison with medical providers
Qualifications:
1+ year experience in nursing preferred
Current RN NYS license
Established patient assessment skills
Knowledge of best-practice standards
Skilled to respond to emergent situations
Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes
Exceptional interpersonal and customer service skills
Proficiency in speaking, listening, reading and writing the English Language is required
