Job Summary and Qualifications

Provides nutritional care to individual patients through assessment of nutritional status and education. Acts as a liaison between patients and Nutrition Services. Acts as a resource for nutritional expertise to physicians and other health care professionals. Provides functional guidance to Dietetic Technicians, Diet Clerks and Food Service Workers. Provides nutritional education to community members via outpatient programs and community events. Demonstrates competency in patient care for neonate, pediatric, adult, and geriatric patients.

JOB SPECIFIC DUTIES:

  • Identifies patients who require nutrition intervention through the use of screening criteria according to protocol.
  • Performs nutritional assessment according to policies and procedures; estimates nutritional needs appropriately based on age, gender, stress factors, diagnosis, weight, intake status, etc.
  • Completes and documents consults for nutrition evaluation/assessment, education and ongoing nutrition intervention according to established standards of practice.
  • Demonstrates competency using the Nutrition Care Process and Nutrition Diagnosis.
  • Documentation is clear and concise.
  • Reassesses patients according to policy to determine the patient’s response to treatment or when a significant change occurs in patient’s condition or diagnosis.
  • Calculates accurately nutritional requirements for patients on tube feedings or receiving parental nutrition and recommends plan of care. Documents in patients plan of care.
  • Demonstrates competency when conduction nutrition counseling with patients and family members. Education method is appropriate for age, cultural and psychosocial needs and living conditions of patient. Counseling is documented in MEDITECH.
  • Attends interdisciplinary patient care rounds, proactively communicating patient's nutritional progress and issues.
  • Assures patient safety through accurate implementation and clarification of diet orders, tube feedings, special feeding instructions, meal trays, supplements, and snacks. Documents and communicates food allergies/intolerances effectively to assure patient safety. Uses approved abbreviations and terminology. Clearly communicates to other disciplines, patients and/or family for safe and accurate nutrition interventions during hospitalization as well as upon discharge home or to another facility or service.
  • Always ensures that each patient receives the best nutrition care possible within dietary restrictions for the nature of the illness
  • Always offers assistance to patient and/or caregiver with menu selections. Offers alternate food selection to patients as requested. Accurately and neatly records current diet prescription in Kardex; includes patients’ food preference, food intolerance, and nourishment throughout the day.
  • Demonstrates competency when calculating diabetic, renal and low sodium diets; is able to make changes in diet exchanges or meal patterns when situation arises.
  • Demonstrates competency when serving as a preceptor for dietetic students.
  • Maintains accurate records of each patient contact. Complete assignments within specified time frame.
  • Performs other related duties as assigned.

What qualifications you will need:
  • (RD-STATE) Registered Dietitian - State must be obtained within 6 months of employment start date
  • (RD/RDN) Registered Dietitian/Nutritionist must be obtained within 6 months of employment start date
