Job Summary and Qualifications To collaborate as a member of the interdisciplinary healthcare team to provide patients of all age groups and acuity levels with high quality Medical Nutrition Therapy following the Nutrition Care Process. Major Responsibilities: Quality Performs timely nutrition screenings and assessments in accordance with the Corporate Screening and Assessment Policy Perform nutrition focused physical exams on patients who are at nutritional risk. • Identify patients at nutritional risk based on established criteria Evaluate age specific, developmental and disease specific nutritional needs of patients • Develop and implement individualized nutritional plan of care. Evaluate and monitor the effectiveness and outcomes of medical nutrition therapy interventions. Identify patient nutrition education needs and provide nutrition counseling to patients, family, and/or caregivers. Participate in the competency assessment process and achieve a passing score of 80% or higher on RD chart audits Adheres to nutrition care guidelines, the nutrition care manual, department/facility guidelines and/or protocols to provide evidence based nutrition care. Integrates the Academy Code of Ethics in all areas of practice. Specialty-Specific Responsibilities Participation in appropriate department and hospital wide committees and community events to promote medical nutrition therapy. Participates in research opportunities and promotes evidenced-based practice Service Assumes accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service. Maintains a patient-first philosophy and engages in service recovery when necessary. Supports the development and implementation of strategies to elevate the patient experience. Displays courteous behavior to customers, guests, and co-worker. Must smile, make eye contact, and use AIDET principles when interfacing with patients. People Mentors and coaches dietetic interns and new employees while fostering their critical thinking skills. Serve as a clinical resource for reliable nutrition information to all interdisciplinary team members, interns, and the community. Growth Pursues goals and professional development to maintain a high level of competency in nutrition care. Finance Perform all duties and uses resources in a fiscally responsible manner. Other Performs other duties as assigned. • Practices and adheres to the "Code of Conduct" and "Mission and Value Statement."
