Leads rehabilitative services in a community-based educational clinic, ensuring high-quality, evidence-based patient care. Supervises physical therapy staff, oversees evaluations and treatment plans, supports patient and caregiver education, and fosters collaboration with multidisciplinary teams. Responsible for leading program development, community outreach, and initiatives that promote functional independence, mobility, and overall wellness.
Major Duties
- Conducts comprehensive evaluations for patients with musculoskeletal and neuromuscular impairments.
- Develops, implements, and progresses individualized Plans of Care focused on achieving measurable functional goals and patient-centered outcomes.
- Maintains compliance by ensuring timely, accurate, and appropriate communication with referring physicians.
- Delivers evidence-based physical therapy interventions and ensures high-quality patient care.
- Maintains accurate, timely documentation that supports medical necessity and complies with regulatory and reimbursement standards.
- Educates patients and caregivers on home exercise programs, injury prevention, assistive devices and technology, and ergonomic safety.
- Collaborates with a multidisciplinary team to ensure holistic patient transitions.
- Leads or participates in community and student workshops, wellness programs, and clinical outreach events.
- Mentors and supervises support staff and students.
- Ensures a safe, clean, and organized clinical environment in adherence to infection control protocols.
- Hire, trains, evaluates and supervises staff and student employees.
Minimum Qualifications
- Doctoral degree in related field from an accredited institution.
- Three years' relevant experience.
- Strong clinical reasoning, problem-solving, and organizational skills.
- Excellent communication and patient engagement skills.
- Demonstrated commitment to community health, health education, and serving underserved and diverse populations.
- Previous managerial experience in outpatient, community, or educational settings.
- Michigan State Licensure.

Physical requirements and working conditions
- Moderate work occasionally working in difficult positions or with physical demands such as continuously lifting or moving materials from 25 to 50 pounds and occasionally lifting or moving materials greater than 50 pounds.
- Work is generally performed in a well-lit, temperature-controlled indoor environment with occasional exposure to the outdoors or any number of elements.
