Job Summary and Responsibilities

Position Summary:

As our Clinical Exercise Physiologist CRT, you will collaborate with a multi-disciplinary team to provide comprehensive lifestyle modification services for individuals with chronic diseases.

Every day you will be responsible for developing and implementing treatment plans for your own caseload.

To be successful in this role, you will actively participate in clinical and program decisions made by the Lifestyle Management Team.

  • Utilizes the AIDET principle to communicate with others, with a focus on patients and their families.

  • Acknowledge Introduce Explain Duration Thank

  • Accurately assesses client status and develops comprehensive treatment plan in collaboration with the Lifestyle Management Team.

  • Establishes patients on clinical pathway

  • Provides monitored exercise therapy to clients in the Lifestyle Management Program (i.e. with diagnosis of heart disease, diabetes, COPD, etc.).

  • Observes, records and reports potentially dangerous signs and symptoms to physician and provides documentation to direct supervisor

  • Adjusts treatment plan in accordance with established protocols, co-morbidities, client goals/needs and medical history.

  • Monitors progress and updates treatment/care plan prn

  • Actively participates in clinical team meetings to review current case load

  • Assists in and directs the education process of clients and their families regarding their disease, process, progress, and treatment plan.

  • Maintains current clinical data, insurance information and program records on each client.

  • Keeps accurate patient medical records and timely entry of data entry in patient chart

Job Requirements

Minimum:

  • Masters Of Science or advanced degree in Exercise Physiology or related field

  • One year experience in lifestyle management, exercise training, prescription and supervision of medical patients

  • Basic Life Support - CPR, upon hire and

  • Advanced Cardiac Life Support, upon hire and

Preferred:

  • Two years experience

  • Certified Exercise Specialist, upon hire or

  • Certified Personal Trainer/Exercise Speciast, upon hire
