Supervised by the Director of Integrated Health and Supported by the Resident Wellbeing Manager as a care team member, the Clinical Case Manager works as part of the Resident Wellbeing Care Team, to provide comprehensive support to resident populations with the goal of lessening behavioral health inequities and improving wellbeing. The CCM plays a critical role in the coordination of clinical services. The Clinical Case Manager provides individual and group therapy, documentation, carries a case load of 10 to 15 internal clients, provides referrals for external behavioral health services and provides after crisis follow up. The Resident Wellbeing Manager manages Resident Wellbeing care teams which include peer specialists, case managers, and clinical case managers, and community resource navigators in the Quality, Innovation and Health department.

What you can expect to be doing in this role at Plymouth Housing

Collaborate with residents, Resident Wellbeing Care Teams Case Managers, and other team members to assess behavioral health needs and develop individualized service plans and overcome barriers to accessing care

Facilitate access and engagement with needed behavioral health treatment, substance use treatment, and medical treatment.

Connect residents with internal and/or external clinical resources as appropriate.

Provide support and advocacy for residents in navigating the healthcare system.

Maintain accurate and timely documentation of resident interactions and interventions.

Engage individuals in a professional manner with a trauma-informed approach.

Ensure accuracy, integrity, and timeliness in the recording of all data within appropriate systems.

Strong clinical skills and experience, organizational and time management skills with high attention to detail

Carry a regular internal caseload of 10 to 15 residents.

Act as a referral source and manage behavioral health care linkages, crisis response and after crisis follow up for residents connected to assigned building Resident Wellbeing Care Team.

Must be able to work efficiently and effectively with customized databases and technology and conduct online research.

Ability to work independently and ability to take appropriate autonomous initiative to conduct study related functions.

Ability to provide and maintain boundaries to help clients and their support in working toward self-defined recovery and improved wellbeing.

Qualifications

Demonstrated ability in providing effective clinical services within a context of harm-reduction practices and voluntary services, and in-depth knowledge of harm reduction practices

Demonstrated ability in providing case management and clinical treatment services to homeless individuals and families, especially those struggling with substance use and mental health issues

Experience in engaging residents, crisis intervention, conflict resolution, and meeting facilitation

Knowledge of supportive housing, housing first, trauma-informed, motivational interviewing and harm reduction principles

Proven ability to navigate King County behavioral health care system and experience in advocating for access to services, including collaboration with other agencies

Excellent computer skills (Outlook, MS Word, Excel, Salesforce or other cloud based database)

·Strong written and verbal skills and the ability to communicate effectively with a diverse population of staff and residents.

Administrative and program experience focused on homelessness, behavioral health, Social Services system preferred.

Education and Licensing

Master's degree in a behavioral science (social work, counseling, psychology)

Licensed (LICSW, LMFT or LMHC)
