    Job Description

BasePoint is seeking social workers who want to grow and specialize in assessment and intake. If you're looking for a role with flexible hours without the burnout, this position may be perfect for you!

    Who We Are

At BasePoint, we believe every teen and young adult deserves hope, healing, and a future filled with possibility. Our mission is to empower individuals and families through compassionate, evidence-based care that is grounded in both clinical excellence and human connection. When you join our team, you become part of a community built on resilience, servant leadership, and the commitment to always ask: “What Would I Want for My Family?”

    What You’ll Do

As a Patient Access Clinician, you will play a key role in creating an environment where patients and families feel supported, safe, and inspired to grow. You will:

    Patient Access Coordination

    • Respond to assigned referrals and inquiries within the PAC Promise standard of 10 minutes during assigned coverage hours.
    • Collaborate with the admissions and other patient access team members to schedule and facilitate same-day assessments where clinically appropriate.
    • Assist in collecting and verifying admission documentation, including consents, insurance information, and identification.
    • Coordinate the handoff from assessment to admission with the receiving clinic team so the patient's clinical picture transfers accurately.
    • Provide outbound referrals to alternate services or providers when BasePoint is not the appropriate level of care.

    Clinical Assessment & Triage

    • Conduct clinical assessments (e.g., risk assessments and consults) to determine clinical triage in scheduling for full biopsychosocial assessment and level of care, including inpatient, PHP, IOP, and outpatient.
    • Identify emergent and urgent needs and prioritize the assessment queue accordingly.
    • Recognize and initiate the need for additional screenings, including substance use, nutrition, abuse and neglect, and developmental screenings.
    • Make appropriate referrals and provide warm handoffs for individuals who do not meet BasePoint admission criteria, including crisis resources where indicated.
    • Apply BasePoint admission criteria and payor medical necessity criteria consistently across assessments.

    Documentation & Authorization

    • Complete and maintain accurate CRM documentation, assessment records, and admission documentation within required timeframes.
    • Document clinical rationale with all received inquiries for recommendation of assessment, risk assessment, consult, and for the level of care recommendation clearly enough to support in clear handoffs, medical necessity, and payor review.
    • Respond to assessment team, patient benefits coordinators, and payor requests for additional clinical information promptly to prevent delays in admission.

    Communication & Collaboration

    • Maintain thorough, professional communication with outreach, admissions, and clinical teams.
    • Serve as a clinical point of contact and clinical resource for internal staff and external referral sources, including schools, hospitals, and community providers.
    • Participate in case consultation and family engagement as needed during the intake process.
    • Communicate with patients, families, guardians, and referral sources within the scope of HIPAA and 42 CFR Part 2.

    Compliance & Quality

    • Uphold HIPAA, 42 CFR Part 2, and all applicable regulatory requirements in documentation and interactions.
    • Maintain a working knowledge of clinical regulations, including Joint Commission standards and applicable state and federal law.
    • Adhere to safety protocols and participate in quality improvement initiatives.
    • Report incidents, near misses, and safety concerns per company policy.
    • Performs other duties as assigned.

Every day, your work will help transform lives—building stronger minds, healthier families, and brighter futures.

    What We’re Looking For

    • Education and Licensure
    • Master's degree in Counseling, Social Work, Marriage and Family Therapy, or a related behavioral health field required.
    • Current Texas license as an LPC, LMSW, LMFT, or LCSW required. Associate-level and supervision-required licensees must maintain an active, approved supervision agreement.
    • CPR certification and Crisis Prevention Training (CPI) required within 30 days of employment and prior to any patient contact.

    Experience

    • 2 or more years in a mental health intake, assessment, or admissions role preferred.
    • Experience across behavioral health levels of care (inpatient, PHP, IOP, outpatient) required.
    • Experience with adolescent and young adult populations strongly preferred.
    • Experience with payor authorization and utilization review preferred.

    Additional Requirements

    • May be required to work flexible hours, including evenings and weekend coverage, to meet referral response and assessment demand.
    • Must pass a background check and any screening required by BasePoint policy, payor contracts, or applicable regulation.
