Job Title: Clinical / Intake Coordinator (Home Care)

We are a growing home care agency seeking a Clinical / Intake Coordinator to support daily operations under the Administrator. This role is responsible for managing new patient intakes, coordinating care services, and ensuring compliance with clinical and regulatory standards.

Key Responsibilities:

• Handle all incoming patient referrals and intake processes
• Verify insurance and coordinate service eligibility
• Communicate with patients, families, and referral sources
• Coordinate caregiver assignments and start of care
• Maintain accurate documentation and ensure compliance
• Support the Administrator with day-to-day clinical operations

Qualifications:

• Prior experience in home care intake or coordination
• Knowledge of insurance verification and authorization processes
• Strong organizational and communication skills
• Detail-oriented with the ability to manage multiple cases
