Description

New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.

New Horizons is seeking for a Clinical Intake Counselor who will be responsible for conducting comprehensive biopsychosocial assessments, developing individualized treatment plans, completing risk assessments, crisis de-escalation and administering mental health and substance use outcome measures. This role ensures clients are connected to the appropriate level of care and linked to relevant community resources. The therapist will work with clients across the lifespan and support both scheduled and walk-in appointments. In addition, this position provides clinical follow-up services for clients discharged from Crisis Stabilization Units and those referred for continued care following medication management appointments. May include rotating coverage for crisis follow-up or coordination with Crisis Stabilization Units.

As a Clinical Intake Counselor, your responsibilities will include:

  • Determine appropriate levels of care based on assessment findings and clinical judgment.
  • Provide referrals to internal programs, external providers, and community resources to support client needs.
  • Collaborate with multidisciplinary teams, including case managers, prescribers, crisis staff, and community partners.
  • Support continuity of care by coordinating follow-up services for clients discharged from Crisis Stabilization Units or referred after medication management appointments.
  • Provide intake services through scheduled appointments, walk-in hours, and same-day access pathways.
  • Engage clients and families in treatment planning and resource navigation.
  • Offer follow-up check-ins to ensure clients remain connected to recommended services and supports
  • Participate in team meetings, case consultations, and clinical individual and group supervision.
  • Maintain up-to-date knowledge of evidence-based practices, community resources, and regulatory requirements.
  • Uphold ethical standards and confidentiality in accordance with professional licensure and agency policy.

Requirements

REQUIRED:

  • Master's degree in Social Work, Counseling, Psychology, or related behavioral health field.

  • Active state licensure or license eligibility

  • Experience conducting clinical assessments and working with diverse populations.

  • Strong knowledge of mental health and substance use disorders, crisis intervention, and

  • care coordination.

  • Excellent communication, documentation, and organizational skills

PREFERRED:

  • Experience in community mental health or crisis services.
  • Familiarity with electronic health records and outcome measurement tools.
  • Bilingual abilities.
