Position Summary / Purpose
The Clinical Liaison leverages strong relationship-building skills and clinical expertise to develop referral partnerships and assess patient eligibility for admission to Schwab Rehabilitation Hospital. This role performs on-site and remote evaluations, coordinates admissions, and ensures seamless transitions for patients while supporting inpatient census and growth goals.
Key Responsibilities
Referral Development & Relationship Management
- Build and maintain strong relationships with referral sources including discharge planners, social workers, case managers, nurses, therapists, and physicians.
- Conduct regular face-to-face visits to cultivate and expand referral networks.
- Identify new referral sources and proactively schedule and conduct outreach activities.
- Coordinate and deliver educational presentations about acute rehabilitation services.
Patient Assessment & Admission Coordination
- Conduct on-site and telephonic pre-admission screenings, including bedside assessments.
- Evaluate patient suitability for acute rehabilitation based on clinical and payer criteria.
- Provide admission determinations within a targeted 24-hour timeframe.
- Complete accurate and timely pre-admission documentation and ensure physician sign-off prior to admission.
- Collaborate with admitting and clinical teams to facilitate smooth patient transitions.
- Ensure patients are medically stable and all necessary records are secured prior to transfer.
Clinical & Operational Collaboration
- Work closely with Schwab's Admitting Department and clinical teams to optimize admission flow.
- Provide alternative care recommendations when patients are not appropriate for admission.
- Demonstrate knowledge of insurance and managed care requirements for post-acute care.
- Support census growth goals and departmental performance metrics.
- Partner with leadership to improve customer service and referral satisfaction.
Customer Service & Quality Improvement
- Deliver exceptional customer service to patients, families, and referral partners.
- Promote teamwork, open communication, and confidentiality.
- Participate in quality improvement initiatives and patient satisfaction efforts.
- Identify process improvements and contribute to department enhancements.
Additional Duties
- Perform other responsibilities as assigned.
Education & Experience
Bachelor's degree (or higher) in one of the following:
Nursing
Social Work
Physical Therapy
Occupational Therapy
Speech Therapy
Athletic Training
Prior experience in a liaison role or working independently preferred.
Experience in post-acute care settings required.
Knowledge, Skills & Abilities
- Strong multitasking, time management, and decision-making skills.
- Excellent communication and relationship-building abilities.
- Proficiency in EMR systems (EPIC and MEDITECH preferred).
- Ability to manage fluctuating workloads and prioritize effectively.
Licensure & Certifications
- Active Illinois professional license required.
- Valid driver's license with no restrictions.
