Good Shepherd Hospice is seeking a compassionate and driven Clinical Liaison (RN/LPN) to help connect patients and families to quality end-of-life care. This role blends clinical expertise, relationship-building, and education to ensure timely and appropriate hospice services.

Responsibilities:
What You’ll Do
Build and maintain relationships with nursing facilities and referral sources
Provide education and in-services to healthcare staff on hospice benefits
Coordinate patient evaluations, admissions, and care planning with physicians and facility teams
Ensure timely follow-up on referrals and eligible patients
Conduct informational visits and support families through the hospice decision process
Use CRM and communication tools to manage your territory and track activity

Qualifications:
What You’ll Need
Active RN or LPN license (in good standing)
Strong communication, presentation, and customer service skills
Ability to build relationships and grow a territory
Experience in hospice, healthcare, or consultative sales preferred
Valid driver’s license with reliable transportation
