    Good Shepherd Hospice is seeking a compassionate and driven Clinical Liaison (RN/LPN) to help connect patients and families to quality end-of-life care. This role blends clinical expertise, relationship-building, and education to ensure timely and appropriate hospice services.

    Responsibilities

    What You'll Do

    • Build and maintain relationships with nursing facilities and referral sources
    • Provide education and in-services to healthcare staff on hospice benefits
    • Coordinate patient evaluations, admissions, and care planning with physicians and facility teams
    • Ensure timely follow-up on referrals and eligible patients
    • Conduct informational visits and support families through the hospice decision process
    • Use CRM and communication tools to manage your territory and track activity

    Qualifications

    What You'll Need

    • Active RN or LPN license (in good standing)
    • Strong communication, presentation, and customer service skills
    • Ability to build relationships and grow a territory
    • Experience in hospice, healthcare, or consultative sales preferred
    • Valid driver's license with reliable transportation
