Clinical - LTSS Service Care Coordinator

Position Title: LTSS Service Care Coordinator

Location: Remote - TX

Duration: 6 months (possibility to extend or convert to FTE)

Schedule: Mon Fri, 8am 5pm, possibility for OT dependent upon business needs

PR: $21- $25-00/hr.

Job Responsibilities:

• Daily check in & communication via group IM(s)
• Daily production logs must be completed and emailed to designated people no later than 9am next business day
• Multi-tasking with multiple monitors and applications
• Utilize work guides and or resources
• 97% Quality (Audits)
• 98% Production
• 0-5% Error XXXX

Job Requirements:

• Education: Bachelor's degree in social work, Health Services, Behavioral Science, or related field, or equivalent experience (2+years) in a managed care environment or working with people who have disabilities Nd/or with vulnerable populations who have chronic or complex conditions.
• Licensure: Required: LPN, LVN, or LCSW preferred. License is the most important.
• Experience: 2 + years in a managed care environment or working with people who have disabilities and/or vulnerable populations who have chronic or complex conditions.
