Responsibilities/Job Description:

Fairview Range is looking for a Clinical Manager to join our Hibbing Home Care and Hospice team.

 

 

The Clinical Manager is responsible for the day-to-day clinical operations of the Home Care and Hospice programs with primary oversight over the Hospice program, ensuring the delivery of high-quality, patient-centered care that is safe, effective, and compliant with all regulatory and accreditation standards. This role provides leadership, supervision, and support to interdisciplinary clinical staff while promoting excellence in patient outcomes, employee engagement, and operational performance. The Clinical Manager collaborates with physicians, referral sources, community partners, and organizational leaders to ensure coordinated care, efficient resource utilization, and achievement of departmental and organizational goals.



Schedule: This position will work 80 hours per two weeks.  The shift length will be 8-hours on the day shifts, with no weekends.

As a Clinical Manager, you will be responsible for:

  • Providing supervision.
  • Facilitating the onboarding/orientation process for new employees.
  • Providing ongoing positive and constructive feedback and coaching.
  • Initiating coaching/performance improvements and disciplinary action in a timely manner.
  • Coordinating performance evaluations in efforts to enhance employee performance and development.
  • Promoting an environment that nurtures professional practice, encourages growth, and helps ensure job satisfaction.
  • Promoting cooperative working relationships with peers.
  • Evaluating learning needs of staff and address learning gaps.
  • Providing timely communication to leader related to patient care and service delivery issues.
  • Addressing and communicating staff concerns, ideas and desired changes regarding standards, practice, procedure, and policy development.
  • Reviewing clinical documentation and ensure completeness and appropriateness.
  • Reviewing timecard oversight and approval. Managing overtime and productivity.
  • Managing operations.
  • Facilitating daily engagement system safety huddles.
  • Overseeing scheduling and work assignments, providing direction, and making adjustments based on patient needs, staff strengths and competencies.
  • Assuring the development, implementation, and updates of the individualized plan of care.
  • Reviewing assessments for accuracy.
  • Managing visit utilization.
  • Managing staff communication and problem resolution to facilitate effective operations.
  • Providing day-to-day coordination of care ensuring organizational policies and regulatory compliance & standards are maintained.
  • Assisting in the development of new programs.
  • Investigating and responds to client/family complaints.
  • Investigating and responds to staff concerns/complaints.
  • Working in conjunction with the Quality Compliance Coordinator to ensure regulatory compliance is met and maintained.
  • Assisting to identify opportunities for improvement and helps develop plans for improvement.
  • Supporting the QAPI program through audits, data gathering, education and role modeling.
  • Providing care coordination and case manager support.
  • Facilitating Interdisciplinary Team (IDT) activities.
  • Implementing and evaluating changes in practice, policies, and procedures.
  • Serving as a resource for the department.
  • Overseeing the referral/intake process to help ensure appropriate admissions.
  • Working with providers and other clinical team members to coordinate patient transitions across the continuum of care to provide seamless transitions for the patient.
  • Evaluating existing processes and problems in workflow looking for ways to improve and streamline practices.
  • Practicing stewardship by using, promoting, and teaching effective and efficient use of resources.


Required Qualifications

  • Graduation from an accredited school of Registered Nursing
  • 5 years nursing experience
  • Management/leadership experience, preferably in Home Care/Hospice
  • MN Registered Nurse (RN) License

Preferred Qualifications

  • Bachelor of Science Nursing
  • Proficiency in Home Care and Hospice Regulations
  • 5 years of experience in home care, hospice, or related setting

 

Qualifications: $64,625.60- $96,304.00 Annual
