Schedule: Monday - Friday
Salary Range: $100,000.00 - $115,000.00
Responsibilities:
Lead and support the Center's clinical team, providing day-to-day guidance, coaching, and problem-solving to help clinicians deliver exceptional patient care.
Oversee daily clinical operations, including new admissions, patient assignments, visit completion, staffing needs, and continuity of care.
Lead daily clinical meetings to keep the team connected, address patient needs, identify barriers, and coordinate priorities. (In person once per week and virtual the other four days)
Serve as a clinical resource for staff, assisting with complex patient situations, care planning, documentation, physician communication, and other clinical needs.
Monitor patient care and clinical documentation to promote quality, regulatory compliance, timely billing, and positive patient outcomes.
Manage and develop team members through regular feedback, joint visits, case reviews, competency assessments, performance evaluations, recognition, and ongoing education.
Partner with Recruiting and leadership to identify staffing needs and participate in interviewing, selecting, onboarding, and retaining clinical team members.
Monitor clinician schedules, productivity, and caseloads while balancing patient needs, staff capacity, and appropriate use of per diem resources.
Support quality and safety initiatives, including incident review, infection control, patient feedback, staff education, and performance improvement efforts.
Collaborate with Administrative, Marketing, and other clinical leaders to support referrals, census growth, efficient operations, and a positive patient and employee experience.
Maintain clinical competency and provide patient or joint visits when needed to support patients and team members.
Qualifications:
Active license as an RN, PT or OT in MA.
Maintain current BLS CPR certification.
Minimum of two (2) years of direct clinical experience; home health preferred.
Prior supervisory experience preferred.
Excellent written and verbal communication skills.
Proficient with computers, tablets, smartphones, EMR systems, email, and other electronic systems.
