Clinical Manager Home Health

We are seeking a full-time Clinical Manager for Seasons Home Health Care who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients.

Seasons Home Health Care strives for excellence and has one of the lowest hospitalization rates in the areas. The person we are looking for enjoys appreciation for their hard work, makes a difference providing quality care and wants to be part of a work family not just another employee.

Work with the physician for medication needs and changes to the plan of care.

Manage other disciplines such LPNs, aides, therapy staff to give the best care needed for each patient.

Ability to complete OASIS, recert, d/c, ROCs documentations within an EMR system

Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.

When appropriate teach and counsel family members about care and processes for disease/injuries.

Attend orientation, meetings and CEUs,

Computer skills; must be able to work within EMR system.

RNs must have current RN Ohio license in good standing.

Reliable auto and current auto insurance.

Current CPR.

Can work well independently
