Job Summary:

Under the direction of the Home Health / Hospice Director, the Clinical Manager is responsible for oversight, planning and directing the daily operation of a clinical team. The Clinical Manager ensures compliance with established professional and accreditation standards, practices and regulatory requirements.  This position is responsible for the support, management and development of direct report staff, ensuring positive team building, effective and productive care management and meeting relevant organizational strategic goals.

Duties & Responsibilities:

Clinical Leadership

  • Provides clinical leadership and maintains current clinical knowledge.
  • Utilizes clinical specialists and organizational resources effectively.
  • Uses reports to manage operations, patient activity, and team performance.
  • Coordinates with scheduling to ensure appropriate patient visit coverage.
  • Manages staff communications and addresses patient and physician concerns, keeping care teams informed.
  • Completes verbal orders and ensures compliance with Medicare CoPs and care requirements.
  • Oversees patient caseloads and ensures continuity and appropriateness of care.
  • Facilitates and participates in team meetings, case conferences, and safety huddles with follow-up.
  • Collaborates with intake, quality, and education to optimize capacity and care delivery.
  • Resolves clinical and operational issues, escalating as needed.
  • Communicates effectively with staff, providers, and leadership.
  • Applies AIDET framework and A+ service recovery principles.

 Quality

  • Ensures compliance with regulatory, accreditation, and documentation standards.
  • Monitors and improves patient satisfaction and HH Compare measures. Regularly communicates patient feedback information to staff.
  • Leads improvement efforts.
  • Reviews documentation concurrently and resolves outstanding issues. Serves as the point person for staff documentation issues.
  • Balances quality care with productivity expectations.

 Development of Staff

  • Recruits, hires, and onboards staff.
  • Develops and implements mentoring and professional development plans.
  • Conducts case conferences and identifies staff education needs.
  • Participates in organizational committees and meetings.

 Finance

  • Ensures productivity, utilization, and capacity targets are met.
  • Manages staffing coverage and resolves scheduling gaps.
  • Oversees supply costs and resource utilization.
  • Completes billing audits/monthly reconciliations and reviews financial performance with leadership.
  • Implements corrective action plans to address issues not meeting budget targets.

Performance Management

  • Completes evaluations and supervisory visits on time.
  • Addresses and documents performance issues in accordance with HR guidelines.
  • Uses data to monitor and manage staff performance.

Hospice Program Manager (in addition to above)

  • Oversees hospice program operations, activities, and interdisciplinary team members.
Requirements:
  • Bachelor's Degree Required, Master's Degree Preferred; Field of Study: Nursing or related health field. Continuing education courses in nursing preferred.
  • Minimum 3 to 5 Years of progressive nursing experience required.
  • Previous supervisory experience required.
  • Experience in a home health care environment required.
  • Rhode Island Registered Nurse licensure.
  • Valid driver’s license and automobile liability insurance is required.
  • Valid CPR certification required.
  • American Nurses Association (ANA) certification preferred.
