Clinical Manager, Indiana (Home Health)Location: Indiana | Full-Time | Clinical LeadershipAbout the RoleOne Home Health is seeking an experienced home health nurse leader to serve as Clinical Manager for our Indiana team. In this role, you will direct daily clinical operations, oversee patient care coordination, and lead a team of nurses, therapists, and home health aides. You will report to our Director of Clinical Services and play a key role in maintaining excellent patient outcomes and full regulatory compliance.What You'll Do

• Oversee all patient care services and clinical personnel within your branch, including staff assignments, care coordination, and referral management
• Supervise RNs, LPNs, therapists, and home health aides, including scheduling, performance evaluations, and staff development
• Ensure clinical practice and documentation meet CMS Conditions of Participation, Indiana home health licensure requirements, and ACHC accreditation standards
• Lead quality improvement efforts and support accurate OASIS and PDGM documentation
• Partner with the Director of Clinical Services on department goals, budgeting, and policy

What You Bring

• Active, unencumbered Indiana RN license in good standing with the Indiana State Board of Nursing
• Minimum of 2 years of nursing experience, including at least 1 year of supervisory or administrative experience
• Prior supervisory or management experience preferred
• Active Illinois RN licensure, or eligibility for licensure by endorsement, is a plus
