Overview
The Clinical Manager, Palliative reports to the AVP of Palliative Care and is responsible for coordinating and managing all day to day business affairs and other clinical activities related to the administration of the palliative division.
Ensures clinical care delivery goals are met and exceeded.
Helps coordinate clinical responsibilities for market.
Ensures scheduling, managing frequency adherence, clinical coordination, medical record maintenance function, and marketing functions are maintained for a team of patients and clinicians.
Manages incoming calls from partners and patients.
Manages Visit Freq compliance, scheduling PRN visits, post hospitalization follow up with patient/family-expediting discharge home with communication with partners, schedule post hospitalization visits.
Facilitates orders for labs, DME, imaging.
Coordinates results with NPs, enters telephone encounters in partner EMR, and enters routine visit med reconciliation in partner EMR.
Ensures transitions to HSP referrals to CTC for care type.
Assists in appropriate identification of HSP eligible pts.
Identifies HH providers that can accept HH referrals from PCC.
Identifies and communicates with local partner office for lab drop off.

Essential Functions of Position
Coordinate and direct all day-to-day clinical activities related to the administration of the palliative program.
Serve as a contact for patient calls and coordinate appropriate follow up for patient needs
Ensure referral process is supported by obtaining sufficient admission and insurance information in order to ensure appropriate and timely admission of patients.
Coordinate and maintain effective and efficient scheduling for the Nurse Practitioners to include geographical considerations that may impact time management and company cost.
Monitor Risk Scoring and ensure visit frequency adherence as indicated by Global Alerts
Reschedule patients with NP, RRN, RSW, AH RN or RN to meet required visit frequencies
Schedule and communicate with NP, RN or AH RN for PRN visit needs
Track ED visits and hospitalizations. Gather required details and medical record documentation surrounding event and document in hospitalization tracker timely and accurately
Schedule post hospitalization/ER visits with appropriate staff, including field and remote teams
Support NPs with submitting orders for DME, imaging, labs as required
Track outstanding results, obtain results timely and provide to NP and PCP as appropriate
Ensure lab and imaging results are uploaded to EMR
Document results given to provider in Telephone Encounter
Coordinate with local partner practice for lab drop off as needed
Update medication reconciliation inside of partner's EMR as appropriate
Assist staff with submitting documents related to the credentialing with payors and maintain a credentialing log.
Manage all daily clinical records functions including establishing and implementing clinical records policies. Ensure clinical record systems are maintained in compliance with state and federal regulations and company policies.
Coordinate information and prepare IDT Agenda for interdisciplinary team meetings and other essential meetings.
Oversee Home Health and Hospice Referral process, ensuring timely acceptance of referrals.
Establish and maintain professional and productive relationships with all referral sources, including the Gentiva family of companies
Act as a resource for customers and community as a whole regarding Palliative Care.
Maintain standard of high quality customer service, and monitor and track referral sources' satisfaction levels. Identify opportunities for additional or improved services to address unmet customer needs through family and physician satisfaction surveys. Communicate customer service issues to the Director as appropriate.
Document any customer complaint utilizing the complaint form and follow up with program director.
Participate, as applicable, with quality assurance and performance improvement activities for program and necessary data collection and reporting.
Implement marketing and promotional initiatives as directed by the Director.
Adhere to and participate in Company's mandatory HIPAA privacy program/practices, Business Ethics and Compliance programs/practices, and all Company policies and procedures, including the Employee handbook.
Facilitate onboarding and training for new employees
Conduct employee evaluations
Adhere to code of conduct conducive to Company policy.
About You
Education/Experience:
Registered Nurse with Home Health, Hospice and/or Palliative Care Experience preferred
Bachelor's degree in Nursing, Business Administration or related field; or equivalent experience and knowledge in Health Care Operations required.
Master's degree in business or related field preferred.
Minimum of 2 years health care administration/operations management required
Certified in Hospice and Palliative Care - preferred
Training/Equipment:
Fluent competence in computer and smart phone navigation.
Working knowledge of, Microsoft Outlook, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.
Must have and maintain reliable transportation to be used for work. Must have and maintain current automobile insurance and valid driver's license.
