Clinical Manager (Home Health Registered Nurse)

Role Summary

Are you a skilled Registered Nurse with home health expertise looking to advance into a high-priority leadership role? We are seeking an analytical, detail-oriented Clinical Manager to lead and support patient care services within our structured home health program in San Antonio, Texas.

Operating in a hybrid capacity, this crucial leadership position focuses on coordinating interdisciplinary care, reviewing patient referrals, and approving clinical orders. You will serve as a key mentor to our field staff, guiding accurate clinical documentation and collaborating with our Quality Assurance/Performance Improvement (QAPI) team. If you are a proactive RN with a strong background in Medicare regulations and clinical systems, this role offers a perfect platform to balance direct clinical leadership with strategic operational oversight.

Duties & Responsibilities

Care Coordination & Case Management

  • Caseload Planning: Coordinate, monitor, and oversee comprehensive care planning for all patients under your assigned caseload to ensure excellent care outcomes.

  • Referral & Order Review: Review incoming patient referrals for clinical appropriateness, and approve/co-sign clinical orders, including LVN and follow-up orders.

  • Interdisciplinary Communication: Facilitate regular case conferences, maintaining seamless communication with physicians, schedulers, therapists, and field staff to support continuity of care.

Documentation, Compliance & QAPI

  • Quality Assurance Support: Align clinical documentation with billing guidelines and coordinate corrections directly with the QAPI team to maintain compliance. (Note: There is no OASIS review required for this position!)

  • OASIS Guidance: Provide strategic guidance on OASIS documentation and assist field Registered Nurses with structured utilization reviews.

  • Regulatory Compliance: Ensure timely submission of assessments and maintain operations in strict accordance with Medicare Conditions of Participation (CoPs).

Team Supervision & Support

  • Supervisory Leadership: Support the Director of Nursing by supervising field staff, assisting with scheduling, and participating in the clinical call rotation and triage responsibilities.

  • Field Visits: Primarily office-based with occasional field responsibilities, including local travel and patient home visits as needed.

Required Qualifications

  • Education: Graduate of an accredited School of Nursing (Bachelor's Degree preferred).

  • Licensure: Active, unrestricted Registered Nurse (RN) license in the State of Texas.

  • Clinical Experience: Minimum of one (1) year of recent clinical experience as an RN.

  • Home Health Expertise: At least one (1) year of dedicated, professional Home Health experience is strictly required.

  • Software Proficiency: Direct, hands-on experience utilizing Homecare Homebase (HCHB) software systems.

  • Regulatory Knowledge: Deep understanding of Home Health operations, Medicare Conditions of Participation (CoPs), and OASIS documentation frameworks.

  • Compliance: Must possess a valid Driver's License and be willing to complete a Candidate Attestation.

Desired Qualifications

  • Experience Level: 5+ to 7 years of overall professional nursing experience.

  • Leadership History: Prior supervisory or leadership experience in a home health or clinical care setting is strongly preferred.

  • Quality Assurance: Experience with utilization review, formal quality assurance, and clinical performance improvement initiatives.

Compensation, Benefits & Perks

  • Total Compensation Range: $105,000 – $142,000 base salary (Commensurate with experience).
