This social worker is a member of a passionate multidisciplinary team providing comprehensive care for pediatric and adult patients in the Cerebral Palsy program. As a valued & highly utilized member of the team, responsibilities include assessment & intervention for barriers to care, acute safety concerns, transition-to-adulthood planning,patient and family coping/stress,and general support as families navigate the healthcare and community systems. Additional responsibilities and opportunities include attending interdisciplinary team meetings, quality improvement initiatives, research, and program development.
