Duties:
Position Summary:
The Clinical Nurse Navigator I is a registered nurse responsible for the seamless coordination of care across the continuum for patients with complex, integration sensitive diagnoses.
The Navigator is a patient advocate who coordinates care in conjunction with the physicians from time of diagnosis or admission through discharge or up to end of life.
Educates patient and family/signicant others on the health care experience; implements, coordinates, monitors and evaluates care options, services and resources.
Links family/patient to appropriate resources as needed.
Proactive in nding ways to enhance the patient experience clinically and in service.
Provides an exceptional patientexperience.
The Nurse Navigator assists in quality improvement initiatives by gathering, tracking, and researching quality metrics.
Monitors outcomes, e.g. patient satisfaction, compliance with treatment regimen and loyalty.
Interacts with department Nurses, may orient and train incoming Clinical Nurses and Patient Care Assistants.
Performs other duties as assigned.
Skills:
Required Skills & Experience:
Five (5) years in nursing and 3 in area of specialty.
Minimum of Five (5) years experience in nursing and Three (3) years of experience in area of specialty.
Neurosciences: Fulfills mandatory stroke education requirements per certification agency.
Oncology: One plus (1%2B) years of Oncology nursing experience
Preferred Skills & Experience:
Acute Rehab: Prior Navigator experience.
Care Management: Prior Sepsis RN experience preferred. Prior experience as a Case Manager, Navigator, or Health Educator.
Hoag Orthopedic Institute (HOI): Prior Orthopedic Navigator Experience.
Community Health: Bilingual skills, especially Spanish are.

- 3 months initially, could go further based on LOA coverage needed.
Is this position remote - No 100% onsite in Costa Mesa.
Work Schedule:
Monday - Friday, 8am - 5pm
Certifications Needed for the Role:
Registered Nurse license. No travel is required for this position.
