Clinical Performance Coordinator

LifeBridge Health | Population Health

Position Summary

LifeBridge Health is seeking a Clinical Performance Coordinator to support our primary care practices in advancing high-quality, value-based care delivery. This role is instrumental in improving patient outcomes through proactive patient engagement, care coordination, quality improvement initiatives, and clinical performance optimization.

As a member of the Population Health team, the Clinical Performance Coordinator partners with providers, care managers, practice leaders, and administrative teams to enhance patient engagement, optimize population health strategies, close gaps in care, improve transitions of care, and increase completion of Medicare Annual Wellness Visits (AWVs).

This position combines healthcare operations, clinical coordination, and data-driven performance improvement to support organizational quality goals while enhancing the overall patient experience.

Key Responsibilities

Population Health & Patient Engagement

  • Identify and conduct outreach to attributed and unaligned patients to establish and strengthen relationships with primary care providers.
  • Promote patient engagement through proactive communication and care coordination activities.

Transitions of Care

  • Review daily Admission, Discharge, and Transfer (ADT) reports.
  • Coordinate timely follow-up appointments after hospitalizations and emergency department visits to support continuity of care and reduce avoidable readmissions.

Quality Improvement Initiatives

  • Utilize payer reports, patient registries, and electronic health record (EHR) tools to identify gaps in care.
  • Partner with providers and clinical teams to improve completion of preventive screenings, immunizations, laboratory testing, chronic disease management, and recommended follow-up care.
  • Support practice initiatives that improve quality performance and patient outcomes.

Annual Wellness Visits (AWVs)

  • Identify Medicare patients eligible for Annual Wellness Visits.
  • Coordinate outreach, scheduling, and completion of Annual Wellness Visits to support preventive care and quality measures.

Documentation, Data Tracking & Performance Reporting

  • Assist providers with documentation practices that support quality measures and value-based reimbursement.
  • Monitor key performance indicators (KPIs), performance dashboards, and population health metrics.
  • Track and report progress on quality improvement initiatives and organizational goals.

Schedule & Work Location

  • Full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m.
  • On-site position supporting multiple LifeBridge Health primary care practice locations throughout the Baltimore metropolitan area.

Qualifications

Education & Experience

  • Associate degree preferred.
  • An equivalent combination of education and relevant healthcare experience may be considered.
  • Two (2) to four (4) years of experience in healthcare operations, population health, care coordination, medical office workflows, quality improvement, or a related healthcare setting.


Pay Range: $19.10 to $28.65 Based On Experience
