Description

The clinical pharmacist is responsible for leading the development and implementation of clinical pharmacy services that enhance the participant experience and support overall organizational performance. This role establishes and maintains a participant-centered pharmacy practice within primary care, focusing on medication management, therapy optimization, and alignment with individual goals of care. The pharmacist works collaboratively with primary care providers, advanced practice providers, center staff, and the pharmacy team to ensure safe, effective, and cost-effective medication use. Additionally, the role supports disease-state management through collaborative practice agreements, contributing to improved clinical outcomes and integrated care delivery.

Core Responsibilities

  • Provide direct patient care focused on medication management, consultation, and education for participants and caregivers - for high-risk individuals, or when addressing medication-related concerns to support understanding, adherence, and participant goals of care.
  • Serve as an embedded clinical partner to primary care providers, advanced practice providers, and interdisciplinary care teams to co-manage medication therapy, support clinical decision-making, and optimize individual participant medication regimens, promoting the safe, effective, and financially responsible use of medications.
  • Lead and contribute to organizational initiatives focused on enhancing medication use practices, advancing quality improvement efforts, and supporting evidence-based, cost-effective therapeutic strategies
  • Lead medication stewardship efforts through deprescribing, reduction of medication burden, optimization of high-risk medication use, and alignment of therapies with participant goals of care.
  • Actively participate in interdisciplinary team (IDT), morning meetings, and other participant-centered clinic meetings to provide medication-related expertise, conduct comprehensive medication reviews, and identify opportunities to improve medication safety, appropriateness, adherence, and therapeutic outcomes.
  • Utilize participant-level and population-based data to identify medication-related risk, prioritize high-risk participants, and inform targeted interventions that improve medication safety, therapeutic outcomes, and quality of care.
  • Serve as the center medication expert, offering drug information and clinical education to participants, providers, and other healthcare professionals.
  • Collaborate with pharmacy operations teams to address medication access barriers, therapeutic substitutions, and participant-specific medication concerns.
  • Participate in medication-related quality improvement, performance monitoring, and practice improvement activities with center leaders, pharmacy operations, and vendor partners, as applicable, to support safe, effective, and compliant medication use, consistent with applicable CMS, PACE, state, and federal requirements.
  • Document all clinical activities, medication reviews, recommendations and interventions in the medical record in accordance with organizational policies and regulatory requirements.
  • Perform additional duties and responsibilities as assigned.

Qualifications

  • Required

  • BS Pharmacy or PharmD degree with active and in good standing pharmacist licensure in state(s) of practice

  • Advanced residency training (PGY1) and/or 3 years of ambulatory care experience

  • Board certification (BCGP, BCPS, or BCACP) within 3 years of hire

  • Preferred

  • Minimum 1-year experience in ambulatory care and/or older adult population

  • Experience working on a multidisciplinary clinical team

  • NM Market: Pharmacist Clinician certification
