Performs a variety of direct and indirect pharmacy services as an integral member of the healthcare team by providing therapeutic consultation regarding application and dispensing of medication therapies in accordance with applicable licensing and regulatory standards. Essential Functions: Selects, compounds, dispenses and preserves drug therapy and other therapeutic agents as prescribed by an authorized medical practitioner in compliance with established standards and age appropriatenessReviews patient chart for drug sensitivities, drug interactions, incompatibilities , dosage and therapy treatment in accordance with patient diagnosis and conditionsMeets with patient, patient family and/or responsible party to verify understanding of medication, potential drug interactions, possible side effects, storage and instructions regarding usage; documents in patient chartOrganizes and maintains drug information sources; responds to questions/requests from medical, nursing and other hospital staffPrepares sterile products for patient administration; determines proper preparation, packaging, sterilization, storage and stability requirements of product.Researches pharmacological information as needed to ensure appropriateness of treatmentPerforms unit dose distribution, intravenous prep, additive and clinical rounds as a member of the healthcare team; responds to emergency situations by assisting with drug therapyServes as therapeutic consultant to medical staff to ensure medication therapy is rationalActively participates in a variety of infection control, patient care policy formulation and drug selection committeesPrepares lectures and demonstrations for members of the Community and staff; assists in training program of Pharmacy studentsPeriodically inspects drugs in clinics and storerooms for adequacy, overstocking, arrangement, storage, proper rotation and evidence of deteriorationPerforms primary care of patients with stable chronic conditions requesting medication refill and patient with acute self-limiting diseases in accordance with established standards of careMaintains stock control records, narcotic records and inventories to ensure compliance with legal and administrative requirementsParticipates in development of department policies, program planning and coordination with other agencies in carrying out pharmacy functionsParticipates in departmental orientation, on the job training and quality assurance programs/initiativesParticipates in a variety of department and hospital educational programs to maintain current skill and competency levels; identifies and discusses performance or training needs with Supervisor Performs other duties as requestedTwo years of clinical pharmacy experience, preferredBachelor of Science degree from an accredited school of Pharmacy AND/OR Doctor of Pharmacy (PharmD)Current and active state unrestricted Pharmacy license, Arizona state license preferred BLS certification or ability to obtain certification within 30 days of hire Indian Preference and Equal Employment Opportunity SCAHC gives preference in hiring to San Carlos Apache Tribal members and other Native Americans in accordance with the San Carlos Apache Tribe's Tribal Preference Policy, as set forth in Section 402 of the Tribe's Human Resources Department Policies and Procedures Manual.
