Diabetes Clinical Pharmacist – Contract

Facility: Lawton Service Unit – Indian Health Service (IHS)

Primary Work Locations:

Duration:1 Year (12-month base contract with up to four option years)

Shift: Days

Schedule: Monday–Friday (Typical 8-hour shifts)

Hours: 40 Hours/Week

Typical Hours:

  • 8:00 AM – 5:00 PM (1-hour lunch)

  • 8:00 AM – 4:30 PM (30-minute lunch)

  • Possible alternate schedules:

    • 7:00 AM – 4:00 PM

    • 9:00 AM – 6:00 PM

    • 1:00 PM – 10:00 PM

  • Select facilities may require weekend coverage (10:00 AM – 6:00 PM)

  • No Federal Holiday work

Pay Rate:$75–$77/hr

Estimated Weekly Pay:$3,000–$3,080/week


Job Summary

The Lawton Service Unit of the Indian Health Service is seeking an experienced Diabetes Clinical Pharmacist to provide comprehensive pharmacy services and intensive diabetes management for eligible American Indian and Alaska Native patients. This position practices under a collaborative pharmacy practice agreement and provides direct patient care, medication therapy management, diabetes education, and medication optimization using evidence-based treatment guidelines.

The pharmacist will utilize the Resource and Patient Management System (RPMS) and Electronic Health Record (EHR) to manage patient care while collaborating with physicians, nurses, dietitians, and other healthcare professionals across multiple outpatient Indian Health Service facilities.


Requirements

  • Graduate of an accredited College of Pharmacy

  • Experience managing patients with diabetes under a collaborative practice agreement

  • Demonstrated knowledge of current American Diabetes Association (ADA) Standards of Care

  • Experience with Continuous Glucose Monitors (CGMs), including placement, interpretation, and medication adjustment

  • Current Continuing Pharmacy Education (CPE) documentation required for licensure

  • Ability to work independently in an ambulatory care setting

  • Excellent patient counseling and communication skills

  • U.S. Citizenship required


Required Credentials

  • Active, unrestricted Pharmacist License in any U.S. state, District of Columbia, Puerto Rico, or U.S. territory

  • Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree from an accredited College of Pharmacy

  • One of the following is required:

    • Board Certified Ambulatory Care Pharmacist (BCACP), OR

    • Board Certified Pharmacotherapy Specialist (BCPS), OR

    • Completion of an ASHP-accredited Postgraduate Year One (PGY1) Pharmacy Residency

  • Documentation of annual Continuing Pharmacy Education (CPE)

  • Annual seasonal influenza vaccination (unless medically exempt)
