Location: Remote (NC residency required)
Pay Rate: $70/hour
Employment Type: Contingent

Why This Role Matters
This role plays a critical part in improving health outcomes for Medicare members by addressing medication adherence challenges and closing key care gaps such as Statin Use in Persons with Diabetes (SUPD).

What You'll Do

  • Conduct outreach to Medicare members to identify and resolve barriers to medication adherence
  • Collaborate with pharmacies and providers to close adherence gaps for unengaged or high-need members
  • Contact providers to address SUPD (Statin Use in Persons with Diabetes) gaps
  • Complete Comprehensive Medication Reviews (CMRs) to optimize therapy and educate members
  • Submit clinical interventions to providers (e.g., drug interactions, dose adjustments, adherence concerns) via fax or CMR platforms
  • Provide medication counseling and education to Medicare members
  • Educate members on pharmacy benefits, including formulary and preferred pharmacy options
  • Offer clinical guidance and support to Pharmacy Quality Specialists

What We're Looking For

  • A current, up-to-date resume
  • Bachelor’s degree or advanced degree in Pharmacy
  • 3+ years of experience in a clinical, retail, or managed care setting
  • Active, current NC state pharmacist license
  • Strong communication and patient engagement skills
  • Ability to collaborate across providers, pharmacies, and internal teams
  • Ability to meet compliance requirements
