POSITION SUMMARY:
The Clinical Pharmacist is responsible for the safe, efficient and cost-effective use of drugs through the application of specialized knowledge, skills and professional judgment. The Clinical Pharmacist serves as a patient advocate when planning and implementing pharmaceutical care by considering the patient’s disease state, age, background, need for understanding and participation in drug therapy in a fiscally responsible manner.
POSITION REQUIREMENTS:- Licensure/Certification/Registrations: Current Registered Pharmacist in theState of Colorado in good standing. Board Certified Pharmacotherapy Specialist Certification preferred.
- Education:PharmD degree in Pharmacy from an accredited College of Pharmacy; with residency or BCPS certification required.
- Experience: 2 to 3 years post-graduate clinical experience.
- Skills & Abilities:Excellent organization and customer service skills. Ability to successfully handle multiple priorities and prioritize appropriately. Must be able to successfully and effectively communicate with a variety of personnel. Demonstrated pharmacist skills commensurate with formal training. Excellent verbal and written communication skills. Skilled in typing and computer applications.
ESSENTIAL FUNCTIONS:- Interpret and screen medication orders; Contact the physician for clarification when needed; Input medication orders into the pharmacy system; Verify and dispense medication orders prepared by the Pharmacy Technicians; Complete medication reconciliation.
- Actively participate in team rounds.
- Analyze in depth patient’s drug therapy for appropriateness and recommend changes to the medical team as necessary.
- Provide direction to support personnel as required by state law.
- Prepare drug monographs for the Pharmacy and Therapeutics Committee.
- Participate in quality improvement activities, assists in maintaining compliance with JCAHO accreditation and other internal and external regulatory standards.
- Counsel patients on the safe and appropriate use of drugs. Coordinate ongoing, proactive processes to evaluate and improve safe medication strategies.
- Develop and present educational programs for Pharmacy staff such as operational changes, journal discussions, in-service presentations, and/or case studies. (Minimum one annually). Provide in-service education on pharmacy related topics to other health care professionals and pharmacy staff at least once annually.

Salary Range: $60.15/ Hour - $86.62/ Hour
