Key Responsibilities: Shift: Days | Rotating Weekends Review, interpret, and accurately verify medication orders and prescriptions in the pharmacy system. Safely compound and dispense medications while ensuring accuracy and compliance with pharmacy standards. Monitor patient medication therapy for drug interactions, allergies, contraindications, dosing appropriateness, and therapeutic effectiveness. Provide clinical pharmacy services including renal dosing, pharmacokinetics, anticoagulation monitoring, pain management, nutrition support, and IV-to-PO conversion. Provide timely medication information and clinical recommendations to physicians, nurses, patients, and other healthcare professionals. Support formulary management and promote appropriate, cost-effective medication utilization. Identify, document, and report medication errors and adverse drug reactions. Maintain accurate documentation of clinical interventions and patient care activities. Collaborate with the healthcare team to optimize medication therapy and improve patient outcomes.
