Position Purpose
The Clinical Pharmacist II is responsible for delivering comprehensive pharmaceutical care across Renown Health acute care services. This includes preparation, distribution, and safe handling of medications, clinical monitoring, and collaboration with healthcare professionals within established guidelines and statutory requirements. Additionally, this position is responsible for attending multidisciplinary meetings, serving as a formal preceptor, or providing departmental staff development opportunities.
Nature and Scope
The Clinical Pharmacist II plays a critical role in delivering comprehensive pharmaceutical care within Renown Health. This position is responsible for both distributive and clinical functions, requiring a balanced approach to medication preparation, dispensing, and therapeutic oversight. The pharmacist ensures medications are handled safely, accurately, and cost-effectively, while also providing expert guidance on drug administration, therapy monitoring, and formulary management.
The scope of this role includes interpreting and verifying medication orders for appropriateness, identifying and resolving potential drug-related issues, and maintaining compliance with all applicable State and Federal regulations and hospital policy. The pharmacist maintains accurate patient medication profiles and documentation, oversees controlled substance records, and ensures timely processing of patient charges.
Clinically, the pharmacist monitors drug therapy, including but not limited to, parenteral nutrition, pharmacokinetics, renal dosing, and IV-to-enteral conversions. They collaborate with providers and nursing staff to recommend appropriate therapies and initiate corrective actions when necessary. Effective communication and documentation of clinical interventions are essential components of this role.
The position also includes leadership responsibilities such as supervising pharmacy technicians and support staff, training interns and students, and contributing to departmental quality initiatives. Additionally, the pharmacist is responsible for providing hands-on training to pharmacist trainees regarding daily shift responsibilities and best practices.
As a key member of the healthcare team, the Clinical Pharmacist serves as a resource to management, supports inventory control, participates in incident reporting, and actively engages in continuous improvement efforts. This position directly contributes to patient care and is integral to ensuring safe and effective medication use throughout the health system.
