Summary:

The Catholic Health Infusion Pharmacy provides infusion pharmacy services across two integrated pharmacy care settings within Catholic Health:
1) SOCH Depew Infusion Center - preparing and supporting chemotherapy, biologics, and other infusions administered onsite.

2) Home Infusion Services - compounding and dispensing IV therapies for patients receiving treatment at home, supported by a partnered nursing agency that provides in-home teaching, line care and infusion support.

As a pharmacist on this team, you will play a critical role in ensuring safe, accurate, and timely preparation of sterile compounded products, including chemotherapy, total parenteral nutrition (TPN), antibiotics, and other infusions. This position requires advanced clinical judgment, precise sterile technique, and strong collaboration with nurses, physicians, and home-care partners. Participation inon-call,holiday, andweekendrotations is required to support continuity of patient care.

Responsibilities:

EDUCATION

  • Graduate from an accredited college of Pharmacy required
  • Licensed in New York State required
  • Current registration in New York State required
  • Pharmacist Doctorate (Pharm D) preferred

EXPERIENCE

  • Demonstrated experience in program and service development
  • Quality assurance and medication use indicator experience required
  • Sterile compounding experience, including hazardous compounding (preferred)Infusion or oncology pharmacy experience (preferred)
  • Demonstrated experience in cost containment programs

KNOWLEDGE, SKILL AND ABILITY

  • Advanced competency in sterile compounding, including chemotherapy and TPN
  • Strong clinical knowledge in oncology, antibiotic dosing/monitoring, and infusion therapy
  • Ability to perform complex pharmacy calculations with accuracy
  • Strong communication skills for patient counseling, nursing support, and provider collaboration
  • Strong supervisory, organizational, and multitasking skills
  • Excellent manual dexterity and attention to detail
  • Ability to work independently during on-call and after-hours situations
  • Ability to operate:
  • Hospital and pharmacy computer systems
  • Compounding devices
  • Automated dispensing machines
  • Prescription balances
