Job Description

We have a new opening for a Clinical Pharmacist at Travis Air Force Base in Fairfield, CA.

Sign-on Bonus: $5,000 payable with first paycheck!

CORE Duties:

  • Precept pharmacy students and residents in a non-supervisory role, when present.
  • Ensure that work areas are clean and safe according to applicable regulations.
  • Understand and enforce compliance with USP 797 and USP 800 regulations to include proper wear of barrier/protective clothing, beyond use dating, and aseptic technique.
  • Understand and ensure compliance with The Joint Commission regulations throughout the facility.
  • Manage patient-specific pharmacokinetic regimens to include vancomycin, anticoagulants, and aminoglycosides.
  • Assess metabolic needs and order therapeutic parenteral nutrition products.
  • Provide medication and healthcare related in-service training to clinical staff on new medications on the market, appropriate use of medications, comparison of current medication therapies, and other topics as deemed appropriate by the department head.
  • Provide clinical direction to pharmacy personnel.
  • Consult with patient care teams through multi-disciplinary rounds and antimicrobial stewardship monitoring to ensure evidence-based patient care. Document pharmacy interventions appropriately in the EHR.
  • Document significant adverse medication events with prescribers, in the patient’s medical record, and in the Patient Safety Reporting system.
  • Document and report patient safety concerns using the Patient Safety Reporting system.
  • Facilitate medical staff drug utilization reviews (DURs) as deemed necessary.
  • Exhibit basic competency in Microsoft Word, Microsoft Excel, and Microsoft PowerPoint and be able to type 40 words per minute.
  • Assess patient’s response to drug therapy and planning drug therapy based on physician- established diagnoses.
  • Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.
  • Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff.
  • Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotic).
  • Administer prescription or non-prescription drugs according to established agreements or MTF protocols.
  • Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting.
  • Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly.
  • Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.
  • Perform a full range of pharmacist procedures.
  • Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
  • Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients.
  • Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Provide medication and healthcare related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
  • Consult with other specialty practitioners to provide optimal patient pharmaceutical care.
  • Document significant medication interactions, and pharmacy interactions with prescribers. Conduct Adverse Drug Reaction (ADR) reporting in accordance with The Joint Commission (TJC) or the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC) standards as appropriate.
  • Facilitate medical staff drug utilization reviews (DURs) as deemed necessary.
  • Assist Outpatient Pharmacy

    Schedule:

    10 - 12 hour shifts, Days (5am- 10pm) and Nights (6pm- 9am), and weekends

    Requirements

    Minimum Qualifications:

    Education:

  • Bachelor's degree in pharmacy from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE), or Doctor of Pharmacy (Pharm.D.)

    Experience: Must have at least one year of recent post-grad hospital-based pharmacy experience in unit-dose distribution and IV additives and/or hospital residency required.

    License:  Active, unrestricted license to practice as an Pharmacist from any US state,

    Life Support Certification: Current AHA or ARC BLS and ACLS Healthcare Provider certificates. 

    Security: Must possess ability to pass a Government background check/security clearance.

    Base salary: $75.00 - $85.00/Hr depending on experience; $96.50/hr as 1099 rate
