Responsibilities:

This position will serve as a decentralized pharmacist in our med/surg areas working collaboratively with nurses and providers in this area.  In addition to traditional pharmacist tasks this role will also attend and participate in discharge rounds, provide clinical review, pharmacokinetic consultation, drug information and actively engage in medication related interventions to ensure optimal pharmacological outcomes for patients.

Qualifications:

Job Requirements

• • B.S. or Pharm D. from an ACPE-accredited School of Pharmacy. 
  • Clinical pharmacy experience in a hospital setting. 
  • Completion of an accredited PGY-1 Pharmacy residency or fellowship.  

  OR 

  • 2+ year’s clinical pharmacy experience with no history of residency or fellowship. 
  • Active Pharmacist license in good standing with the appropriate State Board of Pharmacy. 
  • Other applicable state requirements - i.e. sterile compounding, etc. 
  • BLS & ALS/ACLS certification (BLS required within 90 days of hire; ALS/ACLS required for attending Code Blue). 

Preferred Job Requirements

• Doctor of Pharmacy from an ACPE-accredited college of pharmacy. 
• 1 year of inpatient hospital pharmacy experience
