Vaco is hiring a Clinical Pharmacist
Location: Miramar, FL
Compensation: $120-$140/annually
Schedule: Monday through Friday 9:00 a.m. – 6:00 p.m.
On-call rotation: Approximately one week per month, unless additional coverage is needed due to PTO or other absences
On-call compensation: $300 for the full on-call week + hour works

Key Duties
  1. Identifies and accurately processes orders for appropriate infusion systems and IV supplies for a given therapy.
  2. Collects and organizes all patient-specific information needed by the home care pharmacist to determine the suitability of patients for home care.
  3. Participates in achievement of monthly, quarterly and yearly projections for operations, inventory control, quality assurance, inspectors/audit and staff.
  4. Remains proficient in all areas of pharmacy practice with continuous review of literature and maintaining current files and reference materials.
  5. Maintains standards and policies set forth by the Company.
  6. Accurately compounds and dispenses all therapy types to include but not limited to IV antibiotics & pharmacokinetic monitoring, parenteral nutrition, inotropes, hydration, chemotherapy, pain management, injectables, specialty medications.
  7. Makes suitable recommendations for modifications in the prescribed medication therapy of monitoring parameters for a home care patient.
  8. Addresses and monitors medications specific to quality of life issues.
  9. Modifies pharmacy care plans and discharges summaries as necessary based on evaluation of monitoring data and other pertinent patient-specific information.
  10. Uses a systematic procedure to communicate patient information to and from the home and other patient care settings.
  11. Utilizes standard resources to identify a core library of primary, secondary, and tertiary references appropriate for home care
  12. Assists in developing pharmacy policies and procedures and participate in Quality Improvement (QI) program.
  13. Follows policies and procedures for the proper handling and disposal of hazardous drugs and pharmaceutical waste.
  14. Works with and supervises pharmacy technician's day to day duties.
  15. Speaks to providers and patients upon request of company personnel in a courteous and prompt manner.
  16. Complies with all state, federal, USP<797>, company, and accreditation organization's requirements.
  17. Maintain confidentiality of patient information and company information:
  1. Respect and protect the confidentiality of all information concerning patient care services.
  2. Observe the organizations formal and informal policies for the handling of patient's rights and confidentiality.
  3. Observes the organizations confidentiality agreement.
  4. Follow all procedures as described by the Accreditation Commission for Health Care (ACHC)and the company policies and procedures related to HIPAA.

Other Duties
  1. Participates in the training/orientation of pharmacy personnel and students.
  2. Assists the Pharmacy Management in conducting special projects when necessary.
  3. Assumes on-call responsibilities, Saturday rotation, and other duties as assigned by Pharmacy Management.
  4. Assist in maintaining a clean, safe working environment.
  5. Effectively utilizes software applications to increase efficiency in performing practice responsibilities.
  6. Other duties as assigned.

Qualifications:
  • Graduation from an accredited school of pharmacy, and a current valid Florida License in good standing with the State Board of Pharmacy.
  • Minimum of one year recent experience in hospital or home infusion setting.
  • Required to uphold the principles of compliance as outlined in the Code of Conduct, Employee Handbook and related policies and procedures.
  • Supports and participates in the mandatory Corporate Compliance Program training initiative on an annual or more frequent basis, as required.
  • Proficient in aseptic technique and intravenous solution compounding per USP <797>.
  • Must possess both verbal and written communication skills, interpersonal relationship skills and problem solving skills.
  • Independent thinking, excellent communication, listening, writing, and analytical skills, self-directed, motivated, flexible and cooperative in fulfilling role obligation.
  • Strong organizational, operational and time management skills with the ability to coordinate multiple activities simultaneously.
  • Ability to work independently with minimal direct supervision.
  • Work cooperatively with company personnel and pharmacy personnel.
  • Handle frequent interruptions and adapt to changes in workload and work schedule
  • Set priorities, make critical decisions, and respond quickly to stat requests.
  • Exercise sound professional judgement.
  • Able to fulfill on-call obligations and Saturday rotation.
  • Able to supervise technicians and clerical personnel.
  • Bilingual in English & Spanish a plus.
