Job Title: Clinical Pharmacist
Job Location: Newark, NJ (Remote)
Job Duration: 6 Months (Possibility of Extension)
Shift:8 to 5 standard working hours
Pay Rate: $45.00 - $48.00/hr. on W2

 
Description:
  • The Clinical Pharmacist serves as an internal and external clinical resource responsible for supporting Star/HEDIS quality measures and pharmacy department initiatives.
  • This role focuses on identifying Star/HEDIS measure needs, developing programs, and managing initiatives to achieve 5-Star ratings.
  • The pharmacist will collaborate with health systems, vendors, retail pharmacies, and internal stakeholders to improve clinical outcomes, quality measures, and cost-effective care.
  • The position requires extensive knowledge of Medicare Part D Star programs, HEDIS measures, and healthcare quality improvement initiatives.
Must Have Skills:
  • Active Pharmacist license in the State of New Jersey
  • Strong knowledge of CMS Star and HEDIS quality measures
  • Experience in Health Plan, Medicare, Clinical Customer Service, or Retail Pharmacy Management
  • Project management experience
  • Strong communication, organizational, and analytical skills
  • Proficiency with Microsoft Office Suite, including Excel and PowerPoint.
Responsibilities:
  • Manage outreach efforts and drive performance for Star/HEDIS clinical programs
  • Coordinate activities between the pharmacy department, health systems, and healthcare providers
  • Support Medicare Part D Star programs including Adherence (DM, RAS, Statins), SUPD, MTM, and CMR initiatives
  • Monitor daily clinical activities and ensure achievement of Star/HEDIS measure goals
  • Develop, implement, and evaluate clinical pharmacy solutions to improve quality outcomes and reduce costs
  • Serve as the clinical liaison between the pharmacy department and health systems
  • Conduct member, provider, and pharmacy outreach interventions as needed
  • Track, analyze, and report program performance, patient interventions, and disease management outcomes
  • Collaborate with data teams to develop metrics and monitor quality, cost, and healthcare trends within Medicare populations
  • Maintain program documentation and ensure effective communication across departments.
Required Years of Experience:
  • 2-5 years of Pharmacy experience
  • Experience in Health Plans, Medicare, Retail Pharmacy Management, or Clinical Customer Service preferred
  • Experience with CMS Star/HEDIS quality improvement initiatives preferred.
Education & Licensure:
  • Bachelor of Science in Pharmacy (BS Pharmacy) or Doctor of Pharmacy (PharmD)
  • Current Pharmacist License in the State of New Jersey.
