Novant Health is hiring a Clinical Pharmacist - Outpatient Infusion!
Full-time, Monday - Friday, 8am - 5pm schedule. Remote. NC or SC residency preferred
The pharmacist serves as part of the healthcare team at Novant Health by supporting authentic personalized relationships with patients and their chosen support system, collaborating with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health. The pharmacist accepts responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures.
The IAT (Infusion Access Team) clinical pharmacist serves to deliver high-quality personalized pharmaceutical care for patients receiving outpatient infusion therapy. This includes clinical review of therapy plans to optimize clinical outcomes and accessibility. Clinical pharmacists support therapy plan entry and maintenance to ensure timely initiation of therapy and medication adherence. This role also provides clinical decision support for on-site infusion teams, clinical support for prior authorizations or appeals.
What You'll Do
What We're Looking For
Education: 4 Year / Bachelors Degree, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification. PharmD, preferred.
Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy. Oncology experience preferred for oncology roles, required.
For Infectious Diseases/Antimicrobial Stewardship roles: ASHP - accredited PGY2 Infectious Disease residency OR minimum of 2 years of equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship, required.
Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role, required.
Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
