Clinical Pharmacist Outpatient - Per Diem

TITLE: Clinical Pharmacist, Outpatient - Primary/Ambulatory/Specialty Care

JOB OVERVIEW: The clinical pharmacist is responsible for providing pharmaceutical care services to clinic patients including but not limited to medication therapy management, drug therapy consultation, chronic disease statement management, participation in transitions of care, and provision of patient education and counseling. Additionally, the clinical pharmacist will participate in clinic and pharmacy staff development endeavors, participate in patient care coordination meetings and be an active participant in the clinical practice guideline review and development process.

WORK HOURS: As assigned

REPORTS TO: Department Manager/Supervisor

PREREQUISITES:

Graduate of an accredited College of Pharmacy; PharmD degree strongly preferred

Licensed as a Registered Pharmacist in the State of Washington with license in good standing, or eligible for licensure.

Completion of an ASHP accredited PGY1 residency required or equivalent practice experience. PGY2 residency preferred for specific practice areas (e.g., Oncology).

American Health Association''s Basic Life Support (BLS) for Healthcare Providers - active certification, or required within 30 days of employment

Minimum of 2-3 years of clinical experience in practice setting preferred.

Board certification or equivalent advanced practice specialty highly desirable.

QUALIFICATIONS:

Professional and effective verbal and written communication skills

Strong customer service orientation and placing priority on serving the needs of patients, their families and health care providers.

Demonstrated skill in developing and maintaining positive interpersonal relationships with a diverse population, including the ability to work effectively with the physical and emotional development of all age groups.

Possess knowledge and problem-solving skills related to all aspects of disease management and drug therapy.

Meet credentialing requirements for reimbursement of services by insurance and governmental payers as applicable.

Demonstrate ability to multitask, set priorities and work accurately and independently.

Demonstrate ability to direct and supervise pharmacy residents, technicians, interns, and students.

Participates in and supports professional organizations.

Pass oral and written examinations pertaining to the areas of current VMC clinical pharmacy responsibilities as applicable.

UNIQUE PHYSICAL/MENTAL DEMANDS, ENVIRONMENT AND WORKING CONDITIONS: Requires routine drug education presentations. Requires the ability to continually change priorities in response to changing patient needs. Requires the ability to work in an environment of constant interruptions. Requires the maintaining of a knowledge base of current medications, drug therapy guidelines, and updates to FDA-approved indications. Knowledge of federal and state laws, regulations and standards governing the practice of pharmacy. Precept students and residents. Requires competency in all applicable collaborative drug therapy agreements.
