Summary:
AS THE CLINICAL PHARMACIST, you will be responsible for attending daily rounds, reviewing patient profiles and making recommendations to physicians/extenders to provide pharmaceutical care, pharmacokinetic monitoring is a core element of the daily activities. The Clinical Pharmacist provides decentralized pharmacy services, and serves as a liaison to other departments, precepting pharmacy students and medication education is an essential part of the role.

QUALIFICATIONS:
Graduate From School of Pharmacy, Required
One Year in a Hospital Pharmacy Environment, Preferred
PGY-1 Residency, Preferred or Equivalent Experience in Clinical Pharmacy Practice
Licensed to Practice Pharmacy in the Commonwealth Of Pennsylvania, Required
