The Clinical Pharmacist will collaborate with primary care providers, clinic team members, and pharmacy team members to provide a cohesive care team to benefit the health of patients. The Clinical Pharmacist participates in day-to-day clinical pharmacy operations through collaborative practice agreements. For clinical operations, activities may include, but are not limited to: patient counseling, consultations with provider and nursing teams, treatment adherence reporting and drug monitoring, coordination of long acting injectables, and assisting the prescribers in the appropriate ordering of pharmaceuticals and supplies.
Key Responsibilities
Provision of Direct Patient Care/Chronic Disease Management
Provide and oversee chronic disease state management utilizing collaborative drug therapy management (CDTM) for clinic patients (minimum of 1 patient every hour, total of 8-10 patients a day while on clinic days).
Upon referral, assess patient, adjust medications, screen for drug interactions and adverse effects, provide disease/medication education, and explore opportunities to simplify &/or decrease the cost of the medication regimen.
Clinical pharmacist will utilize CDTM to prescribe medication adjustment, order labs and work collaboratively with patient's primary care provider.
Provide support to patients and providers and promote self-management for chronic disease states and/or pharmacologic management, including but not limited to:
Diabetes
COPD, Asthma
Cardiovascular disease (hypertension, heart failure, hyperlipidemia)
Anticoagulation
Medication reconciliation (regularly scheduled and/or as ordered by provider)
Recommend appropriate vaccines
Implementation, coordination, and administration of long acting injectables for HIV, pre-exposure prophylaxis (PrEP), substance use disorder, etc.
