Clinical Pharmacist
We are seeking a licensed Clinical Pharmacist with 2–3 years of clinical pharmacy experience and an active New York State Pharmacist License to join our hospital-based healthcare team. This full-time position plays a key role in ensuring safe, accurate, and effective medication management while collaborating with physicians, nurses, patients, and other healthcare professionals.
Key Responsibilities
- Dispense medications and pharmaceutical supplies in accordance with hospital policies, procedures, and regulatory requirements.
- Review medication orders for accuracy, appropriateness, and patient safety.
- Collaborate with physicians, nurses, and other healthcare team members to optimize patient care and medication therapy outcomes.
- Provide medication counseling and education to patients and their families.
- Communicate effectively with patients, families, employees, medical staff, and hospital leadership.
- Maintain accurate documentation and support compliance with all applicable pharmacy standards and regulations.
Qualifications
- Active New York State Pharmacist License required.
- 2–3 years of clinical pharmacy experience, preferably in a hospital or acute care setting.
- Strong clinical knowledge and commitment to patient-centered care.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a multidisciplinary healthcare environment.
