    Pharmacy Supervisor – Sentara To Home Pharmacy

    Lead the Future of Outpatient Pharmacy Across Hampton Roads

    Are you an experienced pharmacist looking to take the next step into leadership? Sentara Health is seeking a Pharmacy Supervisor to lead our growing Sentara To Home Pharmacy program throughout Hampton Roads, Virginia.

    This position is ideal for an experienced retail, ambulatory, outpatient, or hospital pharmacist who is ready to transition into pharmacy leadership while remaining actively involved in patient care. You'll work alongside an experienced pharmacy leadership team while helping shape a program that directly impacts patient outcomes by improving medication access, reducing barriers to care, and supporting safe transitions from hospital to home.

    Regional Leadership Opportunity

    This position provides leadership across our Sentara To Home Pharmacy operations serving seven hospitals throughout Hampton Roads. Travel throughout the region is required to support pharmacy operations, staff development, and program initiatives.

    About Sentara To Home

    The Sentara To Home Pharmacy program helps patients transition safely from the hospital to home by providing discharge medications before they leave the hospital.

    Our pharmacists work closely with providers, nurses, case management, and patients to improve medication adherence, reduce delays in discharge, and create a smoother transition home. In addition to medications, the program can assist with home medical equipment and other healthcare needs before discharge, making the transition home as convenient as possible for patients and their families.

    What You'll Do

    As Pharmacy Supervisor, you'll provide both operational and clinical leadership by:

    • Leading daily pharmacy operations across multiple hospital locations.
    • Coaching, mentoring, and developing pharmacy staff while fostering a collaborative, high-performing team.
    • Supporting workflow optimization and operational excellence across the Sentara To Home program.
    • Ensuring safe, compliant, effective, and timely pharmacy services.
    • Participating in all aspects of the medication-use system while providing comprehensive pharmaceutical care.
    • Managing patient drug therapy in accordance with clinical practice standards.
    • Serving as the primary drug information resource for providers and staff.
    • Providing patient education and medication counseling when needed.
    • Leading the implementation of local and system-wide pharmacy initiatives.
    • Developing and maintaining pharmacy policies, procedures, and best practices.
    • Supporting quality improvement initiatives and ensuring regulatory compliance.
    • Collaborating with pharmacy, nursing, physician, and administrative leaders throughout the region.
    • Precepting pharmacy students and residents as assigned.
    • Providing operational support and staffing assistance based on team needs.

    Qualifications

    Required

    • Bachelor's Degree in Pharmacy
    • Active Virginia Pharmacist License
    • Minimum of three years of pharmacist experience

    Preferred

    • Doctor of Pharmacy (PharmD)
    • Ambulatory or Outpatient Pharmacy experience
    • Previous leadership or supervisory experience

    Sentara Leigh Hospital, located in Norfolk, VA, is a 274-bed acute care facility that opened in 1903 and relocated to its present site in 1977. Our hospital completed a renovation in 2016, including two new patient towers, a beautiful atrium, and a multi-story parking garage. Sentara Leigh Hospital includes a dedicated Orthopedic Hospital providing patients access to a full continuum of orthopedic care, from the preoperative phase and surgery to rehabilitation and home care services.
