Position Summary

The Clinical Pharmacist is responsible for ensuring the safe, effective, and appropriate use of specialty medications through comprehensive clinical management services, drug utilization review, patient counseling, and evidence-based pharmaceutical care. This role provides clinical oversight of patient care activities performed by pharmacy technicians and collaborates closely with patients, caregivers, providers, and healthcare teams to optimize therapeutic outcomes.

The Clinical Pharmacist serves as a key clinical resource within the organization, supporting quality improvement initiatives, patient safety efforts, specialty pharmacy programs, and regulatory compliance standards. This position is ideal for a pharmacist who is passionate about patient care, clinical excellence, and improving health outcomes through specialty pharmacy services.

The preferred shift for this position is 11:30 AM to 8:00 PM EST, although schedule flexibility may be required to support business needs.

Essential Duties / Responsibilities

Clinical Pharmacy Services & Patient Care

  • Provide clinical oversight and pharmaceutical care as outlined within the organization's Patient Care Program.
  • Prevent, identify, and resolve medication-related problems through evidence-based, patient-centered interventions.
  • Assess patient therapy plans and provide recommendations to optimize medication effectiveness, safety, and adherence.
  • Identify high-risk patients and perform documented clinical interventions when appropriate.
  • Assist patients with care coordination, medication initiation, and referrals to external healthcare services when necessary.
  • Counsel patients and caregivers regarding medication therapy, disease management, medication administration techniques, adherence strategies, and side effect management.
  • Provide ongoing support at established clinical touchpoints and upon patient or provider request.

Prescription Verification & Medication Safety

  • Verify prescriptions for appropriateness, accuracy, completeness, and regulatory compliance.
  • Ensure prescriptions contain the correct medication, labeling, and supporting documentation according to company and regulatory standards.
  • Perform and document drug utilization reviews for all prescriptions dispensed.
  • Provide prospective, concurrent, and retrospective clinical interventions to promote safe and effective medication use.
  • Demonstrate quality, accuracy, and patient safety throughout the medication distribution process.
  • Report medication errors, adverse drug reactions, and medication recalls in accordance with organizational policies and regulatory requirements.

Clinical Program Development & Quality Improvement

  • Actively participate in performance improvement, quality improvement, and patient safety initiatives.
  • Support the development and implementation of disease management programs, critical pathways, and drug utilization management activities.
  • Contribute to the enhancement of clinical pharmacy services and specialty pharmacy operations.
  • Monitor clinical outcomes and identify opportunities to improve patient care, safety, and operational effectiveness.
  • Maintain current knowledge of specialty medications, disease states, treatment guidelines, and medication delivery systems.

Education & Professional Collaboration

  • Serve as a clinical resource for patients, caregivers, healthcare providers, and internal team members.
  • Provide drug information and therapeutic recommendations to healthcare professionals and care teams.
  • Educate internal staff regarding specialty medications, disease states, and pharmacy best practices.
  • Deliver in-service training and educational programs to healthcare professionals when required.
  • Mentor pharmacy technicians and support their professional development.
  • Collaborate effectively with pharmacists, technicians, providers, and other healthcare professionals to ensure continuity of care.

Leadership & Operational Support

  • Assume Pharmacist on Duty responsibilities in the absence of the Pharmacist in Charge.
  • Support daily pharmacy operations while ensuring compliance with organizational, regulatory, and accreditation requirements.
  • Assist leadership with special projects, clinical initiatives, and operational priorities.
  • Perform other duties as assigned.

Education / Experience

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree required.
  • Active Pharmacist license in good standing with the applicable State Board of Pharmacy.
  • Minimum of three (3) years of clinical patient care management experience required. Clinical rotation experience may be considered for PharmD graduates.
  • Proficiency with Microsoft Office Suite and pharmacy-related software applications.
  • Experience supporting patient care activities, medication therapy management, and clinical interventions.

Preferred Skills / Abilities

  • Experience or knowledge of regulatory, compliance, accreditation, and quality standards applicable to specialty pharmacy operations.
  • Prior experience within Retail Pharmacy, Mail Order Pharmacy, Specialty Pharmacy, PBM, Pharmaceutical, or Managed Healthcare environments.
  • Knowledge of specialty medications, disease state management, and medication adherence programs.
  • Experience participating in clinical program development, quality initiatives, and patient safety programs.
  • Familiarity with healthcare regulations, payer requirements, and pharmacy industry best practices.

Work Environment

  • Hybrid home/office environment with minimal exposure to excessive noise or adverse working conditions.
