Position Summary:

  • The Clinical Pharmacy Manager – Medication Therapy Management (MTM) is responsible for developing, implementing, and leading a comprehensive MTM program across the healthcare system.
  • This role combines clinical expertise, strategic leadership, provider engagement, and business development to improve patient outcomes, reduce total cost of care, expand pharmacist-led clinical services, and generate new revenue opportunities.
  • The incumbent will collaborate with physicians, ambulatory practices, population health, care management, finance, and pharmacy leadership to integrate MTM services throughout the continuum of care while ensuring compliance with regulatory and payer requirements.

 

Essential Responsibilities:

  • Lead the development, implementation, and expansion of a system-wide Medication Therapy Management (MTM) program.
  • Design standardized clinical workflows, policies, documentation standards, and quality metrics.
  • Establish program goals, performance dashboards, and continuous quality improvement initiatives.
  • Recruit, train, mentor, and oversee pharmacists providing MTM services.
  • Clinical Services:
    • Provide comprehensive medication reviews, medication reconciliation, and chronic disease management.
    • Identify and resolve medication-related problems while optimizing medication therapy.
    • Collaborate with physicians and interdisciplinary teams to improve patient outcomes and reduce readmissions.
    • Support transitions of care and ambulatory care initiatives.
    • Provider Outreach, Program Growth & Business Development
    • Develop and execute strategies to expand MTM and pharmacist-led clinical services throughout the healthcare system.
    • Conduct proactive outreach to employed and affiliated physicians, advanced practice providers, and practice leadership to increase awareness, referrals, and adoption of MTM services.
    • Identify opportunities to integrate pharmacists into primary care and specialty practices and develop new clinical service lines.
    • Build strong relationships with providers and operational leaders to support population health, quality initiatives, and value-based care.
    • Identify and implement new revenue-generating opportunities through pharmacist-provided clinical services, payer reimbursement, grants, and other innovative care models.
    • Develop business plans, monitor financial performance, analyze return on investment, and recommend strategies to improve program growth and profitability.
    • Strategic Collaboration Partner with Population Health, Care Management, Ambulatory Services, Medical Group leadership, Finance, Revenue Cycle, and Quality departments to integrate pharmacy services across the healthcare system.
    • Serve as the pharmacy representative on multidisciplinary committees and strategic initiatives.
    • Compliance & PerformanceEnsure compliance with CMS, New York State, payer, and accreditation requirements.
    • Monitor program outcomes, quality measures, patient satisfaction, provider engagement, referral volume, reimbursement, and financial performance.
    • Prepare reports and presentations demonstrating the clinical, operational, and financial impact of the MTM program.

Qualifications

  • Doctor of Pharmacy (PharmD) from an ACPE-accredited college or school of pharmacy.
  • Current New York State Pharmacist license.
  • PGY1 residency required; PGY2 Ambulatory Care or equivalent experience preferred.
  • Board Certification (BCACP, BCPS, or equivalent) preferred.
  • Minimum of five (5) years of progressive clinical pharmacy experience, including ambulatory care, MTM, or population health.
  • Demonstrated experience in program development, provider engagement, and healthcare business development preferred.

 

Minimum Skills, Knowledge, and Ability Requirements:

  • Must be proficient in Microsoft Office, including Word, Excel, Outlook, and PowerPoint
  • Experience in Epic or similar EMR platforms
  • Experience with retail pharmacy prescription processing and claims adjudication 
  • Prior experience on pharmacy dispensing systems and familiar with insurance adjudication processes
  • This role involves working with a broad array of complex disease states and medical conditions. Candidates should be comfortable working with clinical data, lab values, and reading/writing notes in patient medical records
Posted Salary Range: USD $150,000.00 - USD $175,000.00 /Yr. :

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
