Job Summary:

The Clinical Pharmacy Specialist embedded within the Wayne Mobile Health Unit (WMHU) serves as a community-based clinical pharmacist supporting medication access, medication optimization, medication therapy management (MTM), pharmacy operations, and pharmacist-delivered patient care services across the Mobile Health Unit network.

This role supports pharmacist-led clinical services within the Mobile Access Pharmacy Program (MAPP) through operational execution and optimization of medication access initiatives, MTM services, community-based clinical pharmacy services, and pharmacy-related workflows. The position works closely with program leadership to identify opportunities for service enhancement, workflow improvement, and expansion of pharmacist-delivered care.

Job Description:

Essential Duties and Responsibilities:

• Community-Based Clinical Pharmacy Services
  • Deliver pharmacist-led patient care services in community-based and mobile health unit settings.
  • Conduct face-to-face, telephonic, and virtual pharmacist encounters focused on medication therapy management, medication optimization, medication reconciliation, medication adherence, preventive care, and care coordination.
  • Identify and resolve medication-related problems, adherence barriers, affordability concerns, and care coordination needs.
  • Provide patient-centered medication education, disease state counseling, self-management support, and medication-related clinical recommendations.
  • Collaborate with providers and interdisciplinary care teams to improve medication-related outcomes and continuity of care.
  • Provide pharmacist-managed care services under approved collaborative practice agreements, protocols, and delegated authority, including medication initiation, modification, monitoring, and follow-up activities.
  • Support pharmacist-led disease state management initiatives aligned with program priorities, including smoking cessation services and other chronic disease management programs as developed and implemented by the program.
  • Participate in targeted longitudinal patient management activities when appropriate to support program operations, continuity of care, and pharmacist-led clinical services.

• Medication Therapy Management (MTM)
  • Deliver comprehensive and targeted MTM services during MHU events, clinic encounters, and follow-up activities.
  • Conduct comprehensive medication reviews, medication reconciliation, adherence assessments, and medication-related risk evaluations.
  • Develop recommendations to improve medication safety, effectiveness, affordability, and adherence.
  • Identify opportunities for referral to MHU clinical services and pharmacist-led medication management services.

• Medication Access, Pharmacy Operations, and Inventory Management
  • Coordinate medication access services including affordability assessments, manufacturer assistance resources, formulary-based medication coverage programs, medication delivery coordination, and pharmacy partner collaboration.
  • Support implementation and management of medication access initiatives designed to reduce financial and logistical barriers to care.
  • Maintain secure and compliant medication storage practices in accordance with institutional policies, regulatory standards, and program protocols.
  • Manage medication inventory activities including procurement, reconciliation, expiration monitoring, inventory tracking, and distribution processes supporting Mobile Health Unit operations.
  • Coordinate distribution of medications, supplies, and patient care materials utilized by Mobile Health Unit clinical programs.
  • Support operational workflows associated with medications designated for administration by Mobile Health Unit physicians and nursing staff.
  • Identify and resolve operational barriers impacting medication availability, medication access, and continuity of care.

• Clinical Workflow Development & Service Expansion
  • Participate in development and implementation of strategies to improve the efficiency, reach, and effectiveness of pharmacist-delivered services
  • Collaborate with Mobile Health Unit staff to identify patients who may benefit from pharmacist intervention prior to community events and clinical encounters.
  • Assist with development of pharmacist scheduling workflows, pre-visit planning activities, targeted patient outreach initiatives, and referral pathways.
  • Participate in development and implementation of processes that increase utilization of pharmacist-delivered MTM services and medication access resources.
  • Monitor operational metrics and identify opportunities for workflow optimization, quality improvement, and service expansion.
  • Participate in development and implementation of new pharmacy services aligned with pharmacy program strategic priorities.

• Documentation, Operations & Program Support
  • Complete timely, accurate, and compliant documentation within the electronic medical record and designated operational systems.
  • Participate in workflow development, operational planning, implementation activities, and quality improvement initiatives.
  • Support scalability and sustainability of pharmacist-led services across Mobile Health Unit operations.
  • Participate in phone tree coverage, operational support activities, and pharmacy-related workflow coordination.

• Interprofessional Collaboration
  • Collaborate closely with physicians, advanced practice providers, nurses, pharmacists, community health workers, operational staff, and interdisciplinary care teams.
  • Communicate medication-related recommendations, patient care updates, and clinical concerns effectively within interdisciplinary workflows.
  • Serve as a clinical resource for pharmacy learners, residents, interns, and trainees as appropriate.

• Credentialing & Professional Accountability
  • Maintain active pharmacist licensure and required continuing education.
  • Practice within approved scope and delegated authority for all pharmacist-led clinical services and collaborative practice agreement activities.
  • Maintain competency in medication therapy management, medication access services, community-based clinical pharmacy practice, protocol-driven prescribing workflows, and pharmacist-led patient care activities.
  • Maintain competency in disease state management services supported by the pharmacy clinical programs, including smoking cessation and other pharmacist-managed care services as assigned.
  • Maintain required credentialing, training, and competency assessments necessary to provide pharmacist-led services under collaborative practice agreements and institutional protocols.
  • Participate in ongoing quality assurance, protocol training, and clinical workflow review activities as required.

Education, Licensure/Certification, and Training:

Required Qualifications

• PharmD degree from an ACPE-accredited school of pharmacy
• Active pharmacist license in the State of Michigan or eligibility for Michigan licensure prior to hire
• Completion of a PGY1 Community-Based Pharmacy Residency, PGY1 Pharmacy Residency, or equivalent experience with community-based clinical pharmacy services, ambulatory care, medication management, or population health programs. 
• Experience providing medication therapy management, comprehensive medication management, medication reconciliation, or pharmacist-led clinical services.
• Ability to independently manage pharmacist-delivered patient care activities, medication therapy management services, medication access initiatives, and clinical care coordination activities.
• Strong verbal and written communication skills
• Comfort working within hybrid, community-based, and interdisciplinary healthcare environments

Preferred Qualifications

• Experience with medication access programs, affordability initiatives, patient assistance programs, medication procurement, inventory management, medication distribution, or community pharmacy operations.
• Experience providing pharmacist-managed care services under collaborative practice agreements, standing orders, or protocol-driven care models.
• Experience developing clinical workflows, operational processes, quality improvement initiatives, service implementation activities, practice transformation programs, or pharmacist-led service expansion efforts.
• Experience collaborating with community pharmacy partners, public health organizations, community-based healthcare programs, or interdisciplinary care teams.
• Experience working with underserved, underreached, or community-based patient populations.
• Experience with population health outreach, care coordination, preventive care services, social determinants of health initiatives, or non-traditional healthcare delivery models.
• Experience supporting community-based clinical pharmacy services, mobile healthcare programs, public health initiatives, innovative pharmacy practice models, or pharmacist-led community health programs.
• Experience precepting or mentoring pharmacy students, residents, interns, or trainees.
• Familiarity with program development, workflow optimization, implementation of new clinical services, or program evaluation activities.
• Familiarity with electronic medical record documentation, care management workflows, and population health management tools.

Skills and Abilities:

• Strong clinical competency in medication management and MTM services while possessing the operational skills necessary to support medication access infrastructure, pharmacy-related workflows, and community-based care delivery models

• Ability to independently manage pharmacist-delivered patient care activities, medication therapy management services, medication access initiatives, pharmacy-related operational responsibilities, and clinical care coordination activities within established protocols and institutional policies.

Working Conditions/Schedule:

• Hourly position with schedule and hours determined based on operational and programmatic needs of the department, patient care activities, community programming, staffing needs, and operational priorities.
• Responsibilities are performed through a combination of office-based activities at 400 Mack, remote patient care activities, and participation in Wayne Mobile Health Unit community-based operations.
• Some evening or weekend activities may be required based on operational, patient care, quality improvement, or community programming needs.
• Travel between 400 Mack and Wayne Mobile Health Unit community-based sites is expected based on patient care, operational, and programmatic needs.
• Routinely utilizes standard office and clinical equipment including computers, telephones, electronic medical record systems, Microsoft Office applications, and virtual communication platforms.
