The Clinical Quality Coordinator will be responsible to assist in the organization, development, monitoring and management of the Quality Assessment and Performance Improvement (QAPI) program for Interim Healthcare. This role will help support compliance, outcomes, and quality improvement for our Home Care department.
Hours: Full-time | Monday - Friday from 8:00am - 5:00pm | This position may be eligible to work a hybrid schedule after a successful training period to work remote up to 3 day a week.

A few must-haves for this position:
- Licensed Clinician - Registered Nurse (Bachelor's preferred), Physical Therapist, Speech Therapist or Occupational Therapist. License is active and in good standing in the State of Minnesota.
- Graduate of an accredited school for applicable discipline.
- At least two years of home health care experience, including knowledge/completion of OASIS assessments.
- Experience and/or knowledge of performance improvement activities and training/staff education preferred.
- Current certification as an Oasis Clinical Specialist or willing and able to complete this certification upon hire.
Some Functions of what you would do:
- Responsible for the review of Outcome and Assessment Information Set (OASIS) assessments, evaluation of OASIS scoring accuracy, and provision of feedback/education to clinical staff related to OASIS assessments, as needed and according to the agency's written QAPI plan/schedule.
- Assist with electronic submission of OASIS assessment data via Internet Quality Improvement and Evaluation System (iQIES), as needed/assigned.
- Assists in the collection and analyzes data and utilizes outcome analysis to help develop patient and employee education and appropriate QAPI initiatives (PIPs - Performance Improvement Projects) in cooperation with the Clinical Quality Manager and the Management team.
- Advises and provides staff training on the QAPI program, as well as any changes or updates to the QAPI program.
- Provides educational opportunities and in-services to employees related to quality and compliance (Outcome Measures, OASIS, Star Ratings, HHCAHPS, HHVBP, state and federal regulations, Interim policies, clinical standards of practice, etc.) as instructed by the Clinical Quality Manager and/or Clinical Management Team.
