5 days per week schedule

On-call manager rotation

Position Purpose:

The Clinical Rehab Nurse Manager is an RN Nurse who serves in the capacity of a coordinator with assessing, planning, implementation, monitoring and evaluation of the options and services required to meet the residents' health service needs.

Primary Job Duties:

  • Clinical Rehab Nurse Manager Participates in welcoming new residents and their families to the community.
  • RN Initiates and supervises the completion of Physician Certification & recertification for rehab stay.
  • Manages resident and family concerns in a calm & professional manner.
  • Serves as a nursing representative for PAN, Bundle & Medicare meetings.
  • Participates in resident baseline and comprehensive care plan for continuity of care.
  • Preparation, review and submission of insurance documentation, authorization, appeals and audits.
  • Assessing, implementing, and evaluating resident individual level of care needs and ensuring that is reflected in resident care plan.
  • Monitors daily Nursing documentation to ensure resident condition and treatment is being appropriately documented.
  • Participates in the Nurse on Call rotation.
  • Other duties as assigned.

Requirements:

  • Registered Nurse RN license in good standing
  • 3+ years clinical experience
  • Minimum 1 year experience in long term care.
