Title - LTSS Service Care Manager-RN
Duration - 6 months | Possible extension & conversion
Shift - Work schedule 8:00 am 5:00 pm Monday-Friday.
Location - San Antonio, TX and surrounding areas - Bexar County and counties that directly touch Bear County.

Note -
- Training Monday - Friday 8am-5pm CST 6 week course classroom setting CAMERA ON NO TIME OFF DURING TRAINING
- In-home visits up to 3x a week
- Must have reliable transportation
Responsibilities:
We completed 6 member visits per week with 2 days for documentation.
Typically have 3 days with 2 visits per day and 2 full days for documentation.
Member visits completed in member's home so travel from employee's home office to member's home and back is required.
We do work with the pediatric population so we are scheduled visits at 3:00pm which have a great potential (with drive home) to go past 5:00 pm.
This is also a documentation heavy position with specific turnaround times for documentation to be submitted.
Performance metrics are measured by documentation and training compliance as well as set company goals.
