    Join the Novant Health East Cooper Medical Center Team as a L&D Clinical Services Navigator!

     

    Location: East Cooper Medical Center

    Address: 2000 Hospital Dr, Mt Pleasant, SC 29464

    Status: Full Time - 8-5pm

     

     

    Labor and Delivery (L&D) nurse navigators guide high-risk pregnant patients through the healthcare system. They act as the primary point of contact from early pregnancy through postpartum, coordinating care, providing emotional support, and bridging communication between expectant parents and their medical teams.

    What We're Looking For:
    • Education: 4 Year / Bachelors Degree Required.
    • Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.
    • Licensure/Certification: RN, required.
    • Additional Skills/Requirements (required): Ability to successfully complete generic and department-specific skills validation and competency testing.
    What You'll Do:

    Job responsibilities include:

    • Collaborate with the HTC team to support coordinated patient care
    • Coordinate care and communication with providers and pharmacists
    • Triage incoming calls for the designated patient population
    • Provide patient education tailored to population-specific needs
    • Coordinate annual patient visits, including preparation and distribution of materials
    • Assist patients and families with completing paperwork for school and sports participation
    • Ensure timely follow‑up and continuity of care through effective coordination practices
